The North Carolina Division of Water Resources conducted an inspection of the Fish Factory Road WRF
on 11/16/2022. This inspection was conducted to verify that the facility is operating in compliance with
the conditions and limitations specified in Non -discharge Permit No. WQ0005790. The findings and
comments noted during this inspection are provided in the enclosed copy of the inspection report
entitled "Compliance Inspection Report". There were no significant issues or findings noted during the
inspection and therefore, a response to this inspection report is not required.

Inspection Summary:
A routine compliance inspection of the Oak Island WWTF was conducted on November 16, 2022. The purpose of the
evaluation was to verify that the facility is operating in compliance with the conditions and limitations specified in Permit
WQ0005790. Kenny Von Voigt, ORC, and Mark Moore, Wastewater Superintendent, were present during the inspection.
The Town of Oak Island is permitted for a 400,000 gpd, Sequencing Batch Reactor wastewater treatment facility. Flow in the
summer months averages 175,000 gpd and 65,000-80,000 gpd in the winter months. Non -discharge disposal consists of
25.5 acre dedicated reclaimed water utilization system consisting of Fields 6-14 at the William S. Smith Park, Fields 1-3
north of the plant and Fields 4, 4A, 5 and 5A at the plant site. Disposal also includes a 180,000 gpd high -rate infiltration
basin and a conjunctive reclaimed water utilization system at the Oak Island Par 3 Golf Course across the street from the
plant. The conjunctive site has never been used.
In the last two years, the plant has had some treatment issues and violations for fecal coliform, ammonia, TSS and BOD.
Technical assistance was requested from the Wilmington Regional Office in June of 2021 and through emails in May of
2022. Operators state that they believe there has been changes to the characteristics of the influent which is causing
treatment issues. The Town has hired consultants to evaluate and determine the problems and help with solutions.
Currently, additional operational data is being collected in order to make a determination of the exact problem. The Town
continues to update the status of the evaluation and conditions of the plant to the Wilmington Regional Office.
The facility is compliant with Non -Discharge Permit WQ0005790. Please review the comment sections of the inspection

Comment: Influent flows through the manual barscreen before entering the surge tank. The facility has no
grit removal system. The barscreen is cleaned once a day, and twice a day during the summer
months.

Comment: There are dual aeration basins with course -bubble fixed diffusers. The 200,000 gallon basin
was in operation and the 100,000 gallon basin was not during the inspection. The DO was
monitored with the facilities DO meter during the inspection, and the aeration basin DO meter
reading was 0.50 mg/I. The ORC indicated that turning the air supply up to reach a higher DO
will cause solids to float to the top in the downstream clarifier. The Town has hired consultants
to evaluate this issue along with addtional issues that the plant has been experiencing in the
last two years.

Comment: The facility consists of four 53-square foot sand filters that are manually air scoured daily.
Disinfection occurs before the effluent flows through the tertiary filters. The Town wants to
repair the worn concrete walls of the filters in the near future.

Comment: Operation and maintenance records were available and complete, as well as calibration and
bench sheets for the field lab analyses. A review of GW-59 records since 2020 shows Nitrate
limit violations for MW-2 and MW-8 and some TDS violations for MW-5. Staff indicated that the
Nitrate violations could be from the fertilizing activities in that area and will investigate further. A
review of NDMRs since 2021, show multiple limit vioaltions for Fecal Coliform, Ammonia, TSS
and, more recently, BOD. The Town has hired consultants to evaluate and determine the
issues related to these violations.

Comment: The Town is permitted to spray reclaimed quality effluent on the dedicated recreational park
Fields 6-14 down the street, Fields 1-3 just north of the plant and Fields 4, 4A, 5, 5A at the
plant site. Signs are posted to notify the public of reclaimed water use at the recreational park.
The Town is also permitted to operate a conjunctive reclaimed water utilization system at the
Oak Island Par 3 Golf Course across the street, but has never used this site. The unlined
5-day Upset Pond needs to be cleared of vegetation and maintained. Documentation should
be available for review of these maintenance activities.
