NOTIFICATION OF INTENT (NOI) TO CONSTRUCT OR OPERATE INJECTION WELLS
The following are `permitted by rule" and do not require an individual permit when constructed in accordance
with the rules of 15A NCAC 02C.0200 (NOTE: This form must be received at least 14 DAYS prior to injection)
AQUIFER TEST WELLS (15A NCAC 02C .0220)
These wells are used to inject uncontaminated fluid into an aquifer to determine aquifer hydraulic characteristics.
IN SITU REMEDIATION (15A NCAC 02C .0225) or TRACER WELLS (15A NCAC 02C .0229):
1) Passive Injection Systems - In -well delivery systems to diffuse injectants into the subsurface. Examples include
ORC socks, iSOC systems, and other gas infusion methods (Note: Injection Event Records (IER) do not need to be
submitted for replacement of each sock used in ORC systems).
2) Small -Scale Injection Operations — Injection wells located within a land surface area not to exceed 10,000
square feet for the purpose of soil or groundwater remediation or tracer tests. An individual permit shall be required
for test or treatment areas exceeding 10,000 square feet.
3) Pilot Tests - Preliminary studies conducted for the purpose of evaluating the technical feasibility of a
remediation strategy in order to develop a full scale remediation plan for future implementation, and where the
surface area of the injection zone wells is located within an area that does not exceed five percent of the land surface
above the known extent of groundwater contamination. An individual permit shall be required to conduct more
than one pilot test on any separate groundwater contaminant plume.
4) Air Injection Wells - Used to inject ambient air to enhance in -situ treatment of soil or groundwater.
5) In -Situ Thermal Wells (IST) — Used to `heat' contaminated groundwater to enhance remediation.

DESCRIPTION OF PROPOSED INJECTION ACTIVITIES AT THE SITE — Provide a brief narrative
regarding the cause of the contamination, and purpose, scope, goals of the proposed injection activity:
DAA will inject EOS Pro (emulsified vegetable oil) and BAC-9 (bioaugmentation culture) supplied by EOS
Remediation, LLC to stimulate reductive dechlorination of PCE and associated daughter products in deep aquifer
source zone. Two wells will be installed in partially weathered bedrock (PWR)/competent bedrock. Each well
will be intended to be screened from 30 to 40 ft bls to inject in the PWR/bedrock zone. However, the screened
interval of the proposed two wells will be adjusted based on site geology. The estimated total volume of
amendments and dilution water is about 1,590 gallons and chase water is 1,900 gallons. The target flow rate at
each point is between 0.5 and 1 gallon per minute (gpm) which was estimated from previous injection activities
(i.e., field -observed injection rates and subsequent performance monitoring). Amendment will be injected in up
to two injection locations for a total flow rate between 0.5 and 1 gpm resulting in an estimated injection duration
of about six days.

NOTE: Only injectants approved by the epidemiology section of the NC Division of Public Health, Department
of Health and Human Services can be injected. Approved injectants can be found online at
http://deq.nc.gov/about/divisions/water-resources/water-resources-permits/wastewater-branch/ground-water-
protection/ground-water-approved-injectants. All other substances must be reviewed by the DHHSprior to use.
Contact the UIC Program for more info if you wish to get approval for a different additive. However, please

L. MONITORING PLAN — Describe below or in separate attachment a monitoring plan to be used to determine
if violations of groundwater quality standards specified in Subchapter 02L result from the injection activity.
Baseline sampling will be conducted after two injection wells are installed and developed, prior to the injections.
Each newly installed injection well pair will be sampled for Volatile Organic Compounds (VOCs). One
monitoring well (MW-52D) near the injection area will also be sampled for VOCs, total organic carbon (TOC),
sulfate, nitrate, total and dissolved iron and manganese, alkalinity, methane/ethane/ethene (MEE) and molecular
biological tools (MBTs) . Approximately three months following the injection test, monitoring well MW-52D
will be sampled for VOCs, TOC, sulfate, nitrate, total and dissolved iron and manganese, alkalinity, MEE and
MBTs. One newly installed injection well will also be sampled for VOCs.
Prior to sampling, water levels will be gauged at each of the wells. The wells selected for sampling will then be
purged with a peristaltic pump and one -time -use new tubing using low -flow purging and sample collection
procedures. During well purging, the water quality indicator parameters pH, temperature, dissolved oxygen
(DO), oxidation-reduction potential (ORP), conductivity, and turbidity will be measured at three to five-minute
intervals until these parameters stabilize within specific ranges. Following purging, groundwater samples will
be collected and submitted to a North Carolina certified laboratory for VOCs by EPA8260D, TOC by SM 5310B,
alkalinity by SM 2320B, nitrate by HACH 10206, sulfate by EPA 9056A, total and dissolved iron and manganese
by EPA 6010D, and MEE by RSK 175 and to Microbial Insights, Inc. in Knoxville, TN for MBTs by CENSUS
analyses.

Keep containers tightly closed in a cool, well -ventilated area. The DHC
microbial consortium can be supplied in stainless steel kegs designed for
maximum working pressure of 130 psi and equipped with pressure relief
valves. The kegs are pressurized with Nitrogen up 15 psi. Do not exceed
pressure of 15 psi during transfer of DHC microbial consortium from kegs.
Do not open keg if content of the keg is under pressure.
BAC-9 may be stored for up to 3 weeks at a temperature range of 2-4°C
without aeration. Avoid freezing.
