SUBJECT: Compliance Evaluation Inspection
NC General Wastewater Permit NCG510000 for Discharge of Wastewater from a
Groundwater Remediation System
Certificate of Coverage: NCG510536 for the Former T&G Grocery, Graham,
Alamance County, North Carolina
Dear Mr. Pritchett:
Mr. Jim Gonsiewski of the North Carolina Division of Water Resources (DWR), Winston-Salem
Regional Office (WSRO), visited your facility on November 2, 2022 and met with Mr. Danny
Oldham, Remediation Technician/Site Supervisor of Pyramid Environmental to perform a
compliance evaluation inspection of your facility's groundwater remediation system. Details of that
inspection are included on the attached EPA Water Compliance Inspection Report.
The permit expired on September 30, 2020. At the time of the inspection, our records indicated that
the annual fee for 2021 has not been paid. The annual permit fee is $100 per year. Once this fee is
paid, the permit will be automatically renewed. Please pay this permit fee and ensure in the future
that the fees are paid in a timely manner.
The following items were noted during the inspection:
The remediation system at the site has been operating intermittently since May 2017 based on
directives by the NCDEQ Underground Storage Tank (UST) section. The system has been operating
since June 2022 according to Mr. Oldham. Currently twenty-three (23) wells are located on and
off site (sixteen (16) monitoring wells, two (2) replacement monitoring wells (for wells that went
dry) and two (2) recovery wells). Additionally, three (3) soil vapor extraction (SVE) wells were
installed on the site. The monitoring wells and effluent are sampled quarterly. The system effluent
was last sampled on September 13, 2022. Laboratory analytical results for this sampling event
indicated all analyzed constituents were below laboratory reporting limits. Currently one recovery
well is used in the remediation system, which treats the groundwater with a combined pump and
treat and soil vapor extraction system. The remediation system and the building in which it is located
appear to be in good condition.

The outfall for the 2-inch PVC effluent pipe that transports treated water from the remediation
system is located southwest of the site. This outfall empties into an unnamed tributary to Haw Creek.
