received September 6, 2022. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on December
30, 2022.

rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through H, or failure to provide the additional information on or before the above requested

E. Residuals Source Certification (FORM: RSC 06-16) Form:
1. This application includes the modification of the Onslow County Water and Sewer's Swansboro
WWTF (Permit No. WQ0023261) to increase their dry tonnage. A permit modification for another
facility recently indicated that they were going to be handling this facility's residuals. Please clarify
whether this source is still to be maintained as a part of this permit.
2. A hazardous waste determination for the Bald Head Island WWTP (Permit No. WQ0000193) was
not provided as required in Item II.3 of the RSC form. The note for exemptions for 100% domestic,
<0.5 MGD facilities in this section is only for facilities that are non -municipal but the Town of
Bald Head Island is a municipal entity. This hazardous waste determination section shall be
completed for this source's residuals, and the laboratory analytical reports shall be provided.
