Division personnel have reviewed and approved your request to transfer ownership of the subject permit, received
on March 21, 2014. This permit modification documents the change of ownership.
Please find enclosed the revised permit. All other terms and conditions contained in the original permit remain
unchanged and in full effect. This permit modification is issued under the requirements of North Carolina General
Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency.

TO DISCHARGE WASTEWATER UNDER THE
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
Water Quality Utilities, Inc.
is hereby authorized to discharge wastewater from a facility located at the
Yonahlossee Resort 86 Club
NCSR 1102 south of Boone
Watauga County
to receiving waters designated as Lance Creek in the Watauga River Basin in
accordance with effluent limitations, monitoring requirements, and other conditions
set forth in Parts I, II, III and IV hereof.
This permit shall become effective May 01, 2014.
This permit and authorization to discharge shall expire at midnight on September 30, 2017.

SUPPLEMENT TO PERMIT COVER SHEET
Water Quality Utilities, Inc., is hereby authorized to:
1. Continue to operate an existing 0.04 MGD wastewater treatment system
consisting of the following components:
• Aerated flow equalization basin
• Three-way splitter box
• Aeration basin
• Clarifier
• Aerated sludge holding tank
• Disinfection / post -aeration chamber with UV disinfection
• Continuous flow -recording device
This facility is located at the Yonahlossee Resort and Club off NCSR 1102 (Shulls
Mill Road) south of Boone in Watauga County.
2. After receiving an Authorization to Construct from the Division, construct and
operate a 0.150 MGD extended -aeration package plant with sludge holding.
2. Discharge from said treatment works at the location specified on the attached
map into Lance Creek, classified C-Trout waters in the Watauga River Basin.

A. (1.) EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTS (0.04 MGD)
During the period beginning on the effective date of the permit and lasting until expansion above 0.04
MGD, the Permittee is authorized to discharge from outfall 001. Such discharges shall be limited and monitored by
the Permittee as specified below:

MINUTES OF AN ACTION WITHOUT MEETING
OF
THE SOLE DIRECTOR AND SHAREHOLDER
OF
WATER QUALITY SERVICES, INC.
An Action without Meeting of the sole Director and Shareholder of Water Quality
Services, Inc was duly conducted pursuant to the applicable North Carolina Statutes.
The purpose of this Action without Meeting was to consider approving the sale of the
assets and the realty for the Water Treatment Plants at Yonahlossee and Willow Valley to
Water Quality Utilities, Inc . The sole Director and Shareholder duly waived notice of
such meeting as attested to by his signatures below.
After much discussion and deliberations the following Resolutions were unanimously
adopted:
BE IT RESOLVED that the Corporation is hereby authorized to sell both the assets and
the realty for the Water Treatment Plants at Yonahlossee and Willow Valley overunto
Water Quality Utilities, Inc for $150,000.00.
BE IT FURTHER RESOLVED the appropriate corporate individuals are authorized
and empowered to enter into and execute whatever documents are necessary to effectuate
the sale and transfer such as Deeds, Bills of Sale, Assignments, Prorations, etc.

it has in its Customer Contact List overunto Water Quality Utilities, Inc.(See Ex. A
Attached hereto)
By this Assignment Water Quality Services, Inc will have no further interest in the same.
