FACILITY OVERVIEW:
This facility treats domestic waste from a portion of Stokes county near Danbury and serves a population of
183. The facility has a permitted and as-bult flow of 0.1 MGD. The current treatment system includes the
following wastewater treatment components:
• Manual bar screen
• Aerated flow equalization chamber with
dual pumps
• Dual aeration basins with three blowers
• Dual hopper bottom clarifiers
APPLICATION
• Aerobic sludge digester
• 3 module ultraviolet disinfection system
• Flow measurement & recording
• Duplex submersible effluent pump station
The application does not indicate any changes in the facility or treatment since the last renewal of the permit.
The application cover page notes that the outfall location has been updated in the application since it was
previously indicated in the wrong location. The treatment system is operated by Stokes County. The annual
average daily flow is indicated as around 0.04 MGD over the last three years, so the facility is operating below
its designed and permitted capacity. The facility uses Pace Analytical for weekly operation, sampling and
notification to owner.
INSPECTION
The most recent NPDES inspection occurred on February 1, 2022. Composite samplers were down and grab
samples were being collected instead. The inspector noted that solids were making their way through the plant
and the permittee needed to mitigate the source of solids at the influent. There were also issues noted with the
UV system needing to be fully equipped and operational and that there needed to be adequate parts on hand to
minimize impacts.
The last inspection under the Deemed Collection System Permit (WQCSD0253) was September10, 2020, and
there were no violations.
COMPLIANCE:
During the permit cycle this facility has had four (4) NODS and thirteen (13) NOVs. The violations have been
largely for Fecal and TSS issues. There have been two (2) enforcement actions over the permit cycle. The
permittee paid a total of $911.65 with penalties of $700.

The data reviewed for the renewal is from March 2017 through June 2022.
REVIEW OF PERMIT LIMITS AND PARAMETERS OF INTEREST:
Flow - The facility is operating on average at around 34% of the as -built flow allowed by the permit. Only
sixteen of the 64 monthly values were at or above this 0.034 MGD level.
BOD - No limit violations for BOD and the average BOD removal was 97%.
TSS - There were six TSS limit exceedances. Overall TSS removal averaged 94%.
Fecal Coliform - There were thirteen (13) weekly average exceedances of fecal limits. Note above that the last
inspection noted a need for better operation of the UV disinfection equipment.

Recommended chlorination or UV disinfection. No parameters water quality limited but could impact
future allocations. Original Issuance date of the permit noted as February 26, 1993.
June 28, 1994 - Authorization to Construct (ATC) for construction of 0.10 MGD extended aeration pacakage
plant with clarifier and sludge return.
January 22,1997 - Permit Renewal - Notes that the facility began discharging in late 1995 and that
parameters are effluent limited due to the assimilative capacity of the receiving stream. Some monitoring
changed to weekly due to the classification as a Class II facility. Renewal removed a few special conditions
that no longer applied and set limits as

BOD and TSS also included requirement for 85% pollutant removal. The permit included monitoring for
NH3-N, Temperature, TN and TP with no instream monitoring required.
• May 15, 2002 - Permit Renewal - TRC changed to 2/week and temperature to daily due to 213.0500
standards. There is information in the file about a 1999 request from the Stokes County Soil and Water
Conservation District requesting to use part of the discharge in a constructed wetland demonstration
project. There is no further information on this project.
• May 22, 2007 - Permit Renewal - No changes.
• April 18, 2012 - Permit Renewal - Added TRC footnote on compliance threshold.
• January 6, 2017 - Permit Renewal - eDMR language added to the permit. Some compliance issues noted in
the fact sheet.
PROPOSED PERMIT CHANGES:
• Updated eDMR requirements to be consistent with final EPA rule.
• Added regulatory citations throughout permit as needed.
• Permit map information updated including location of outfall and WWTP and coordinates for outfall as
provided in the application.

The permit looks great! The only things I caught are the following:
-'Director' is mentioned twice on the signature line of the first page of the permit
-There's an extra '03' in the HUC code on the permit map
I also wanted to mention that the renewal application in your folder is only one page, just so you know! I pulled the full
application from Laserfiche to check everything, but everything looks good! Great job!

Not necessarily issues with the renewal. But the last inspection our office did in February indicates composite sampler
failed and flow meter not calibrated and some other issues. I would like to have someone follow up on those issue,
because that inspector is no longer with us and never followed up, obviously. No need to hold off on renewal though.
