FACILITY OVERVIEW:
This facility treats domestic waste from a single-family residence. The home is three bedroom and the design is
for 360 GPD discharge as allowed (120 GPD per bedroom).
Property has an existing sand filter system that appears to have been previously approved by the Durham
County Health Department in 1978. The discharge has never been permitted by DWR. This application will
work to correct the unpermitted issue and to construct an improved treatment system with no increase in flow.
The system will discharge from the permittees property in the same location as the existing system to a UT to
Ellerbe Creek in the Neuse River Basin.
PROPOSED IMPROVEMENTS AND CONSTRUCTION:
The existing system will be replaced with the following treatment system
• 1,250 Gallon Septic Tank with Effluent Filter
• AdvanTex AX-20 RT Pod system with re -circulating chamber
• UV Disinfection
• Control Panel With Alarm
• Discharge Point with Riprap
HYDRAULIC TRESPASS
The UT is located adjacent to the permittees property so discharge will be directly to the UT. No issues with
Hydraulic trespass.
STAFF REPORT
Staff report completed by Jane Bernard (RRO) on October 25, 2022. The regional office noted in the staff report
that this was an existing, properly functioning, unpermitted sand filter system that was being upgraded. The
region also indicated that there were no onsite alternatives to the discharging system. The discharge was noted
as being directly off of owner's property into the UT to Ellerbe Creek. The Regional Office recommended
approval of the permit.

this is an existing system with no alternatives. The applicant will

This is the cheapest proposed system but is also the only technologically
feasible alternative.
LOCAL GOVERNMENT REVIEW FORMS
The application package included copies of the forms submitted to the City of Durham and the County of
Durham but did not include completed forms or information to verify that the local governments had received
the forms. The applicant and consultant were sent and add info email on October 25, 2022, about this issue.
Final issuance will wait for response on this issue.
OTHER NOTES:
Plan sheet WW-4 - System Details #1- notes that setback in compliance with the requirements, so no waiver of
setback distances is required.

Everything looks great! I love the format of your factsheet. Very easy to read. I might copy it!
When I'm uploading final documents to LF, I have two different documents. This might not be the best way to do it
though, it's just how I've been going about it:
1. Issued COC/AtC (so the cover letter and the actual COC, plus the map)
2. Copy of the approved plans and specifications (so the large blueprint drawings and specifications)
I don't include a copy of the NCG55 with the COC when I upload it to LF. I only send a copy to the permittee.
Same with the engineer's certification. I only upload that when I get the completed form back.

I have a SFR permit ready for review. This is the first one of these I have done so I'm guessing there will be a number of
comments. I wasn't sure how much of the plans and specs to include. Maybe some of the calculation sheets don't need
to be included. Let me know what you think. I have some questions about the final process. I noticed in LF that some
people include the whole NCG550000 permit in the document they upload. I was thinking that might not be needed. I
don't think you had included it in one that I looked at that you did.

Hey Jane,
I did copy Kevin on those emails. What I was asking them to provide is the forms where the city and county have to
respond about whether the permit is in compliance with their ordinances. By statute we have to have them respond to
that. These are in the application starting on page 81 but just a copy of what they sent the local governments. Looks
like our form requires that if there is no response, they have to verify that with copies of the signed certified mail
card. They did not send those in unless I missed them. The information on page 15 is where they are saying the soils are
unsuitable. I haven't done too many NCG55s, so if my understanding of this is off let me know so I can respond back to
them.

I am the permit writer for the Single Family Residence NPDES permit application for the property and 2219 Clements
Drive, Durham NC — NCG551791. I am currently reviewing the application information and developing the draft
permit. I wanted to touch base about a couple of issues, they are outlined below.
First —Additional Information Request
Local Government Review Forms — In your application you provided the letters that had been sent to the City of Durham
and Durham County and a copy of the forms you sent to them. No additional information was included to show they
had completed the forms or failed to respond. As noted in the Instructions to the Applicant on this form, we need to
receive either the forms as completed by the local governments or documentation that the forms were received by the
local governments. The form notes that if the local governments fail to respond this can be documented by providing
the Division with copies of the certified mail card and a notarized letter stating the local governments failed to respond
in the 15-day period.
You can provide this information to me through email if you would like.
Second — Information on Electronic Submittal of Permit Documents
In order to provide more convenience, control, and security to our permittees and assist them in processing their
transactions, The Division of Water Resources is currently transitioning towards electronic correspondence. This will
hopefully provide more efficient service to our permittees and other partners and will allow us to more effectively
process and track documents. We are writing to ask you for your approval of the transmittal of documents related to
your permitting and related activities with the Division in an electronic format. Documents will be emailed to the
appropriate contact person(s) in your organization in a PDF format.
Please respond to me through email with verification that transmittal of your documents in an electronic manner is
acceptable to you. If you have any questions, please feel free to contact me.
