Dear Mr. Testa:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
This final permit includes one major changes from the draft permit sent to you on May 7,
2008:
➢ A new Total Residual Chlorine compliance level (50 AWL) has taken effect for all NPDES permits. A
footnote noting the new compliance level has been added to the Effluent Limitations Sheet for
outfall 001.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings
(6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this
decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local
governmental permit that may be required. If you have any questions concerning this permit, please

08-05 of the Broad River Basin in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective September 1, 2008.
This permit and authorization to discharge shall expire at midnight on August 31, 2013.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby revoked. As of this
permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive
authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and
provisions induded herein.
Roadside Truck Plaza, Inc. is hereby authorized to:
1. Continue to operate an existing 0.0145 MGD extended -aeration wastewater treatment
system discharging via outfall 001 with the following components:
• Two grease traps
• Splitter box
• Two manual bar screens
• Two aeration basins with diffused air
• Two secondary clarifiers with RAS / WAS lines
• Chlorine contact chamber
• Dechlorination
• Two sludge holding tanks
• V-notch weir with flow recorder
The system is located at the Roadside Truck Plaza WWTP, south of Kings Mountain off
Interstate 85 at Dixon School Road in Cleveland County.
2. Continue to operate an existing 0.005 MGD wastewater treatment system discharging via
outfall 002 with the following components:
• Oil/water separator
The system is located at the Roadside Truck Plaza WWTP, south of Kings Mountain off
Interstate 85 at Dixon School Road in Cleveland County.
3. Discharge from said treatment works at the locations specified on the attached map into
Dixon Branch (outfall 001) and Kings Creek (outfall 002). Both streams are currently
class C waterbodies in subbasin 03-08-05 of the Broad River Basin.

(or mixing with) the receiving stream.
2. All volumes of wastewater discharged from this outfall shall be monitored. If continuous flow monitoring is not
feasible, the Permittee shall record the approximate time that the discharge began and ended along with the
instantaneous flow at the time of sample collection.
3. The discharge from this facility shall not cause turbidity in the receiving stream to exceed 50 NTU. If the
instream turbidity exceeds 50 NTU due to natural background conditions, the discharge cannot cause turbidity to
increase in the receiving stream.
This permit does not limit stormwater discharges uncontaminated by any industrial/commercial activity.
This permit does not limit stormwater discharges that are untreated (not discharged via oil/water separator
or any other treatment mechanism).
There shall be no discharge of floating solids or visible foam in other than trace amounts.

PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

e. Description of existing or substantially constructed WWT facilities: The existing
WWT facilities at outfall 001 consist of two 900 gallon capacity septic tanks acting
as grease traps followed by a flow splitter box, pH adjustment using powdered soda
ash, two 0.0075 extended aeration package type WWTPs connected in parallel
containing bar screens, aeration and clarification, two final settling tanks on each
plant (originally used for disinfection), a common chlorine contact tank with sodium
hypochlorite (HTH) disinfection, a de -chlorination unit, a flow recorder with totalizer
and V-notch weir, and one (1) aerated sludge holding tank.
This facility also has an oil/water separator, which collects and treats SW from the
fuel loading area, and discharges through outfall 002.
f. Description of proposed WWT facilities: There are no proposed WWT facilities.
g. Possible toxic impacts to surface waters: N/A

2. Residual handling and utilization/disposal scheme: The permittee currently utilizes Wesson
Septic Tank Service and dispose of the residuals at the Cliffside Sanitary District in
Rutherfordton County North Carolina.

PART IV - EVALUATION AND RECOMMENDATIONS
The permittee, Roadside Truck Plaza Inc., is requesting renewal of the subject permit to
discharge treated domestic and industrial wastewater from an interstate car and truck restaurant and
refueling plaza. The WWT facilities appeared to be functioning properly at the time of the site visit.
De -chlorination unit has been added since the last renewal.
Pending review and approval by the P& E Group, it is recommended the Permit be renewed
as requested.

This package includes information for renewal of the Roadside Truck Plaza, Inc.
wastewater treatment plant permit number NC0032867. This permit includes two (2)
separate discharge points which are described as Outfall 001 and Outfall 002. The
following is a brief summary for each.
OUTFALL 001
This facility has a permitted flow of 0.0145 MGD, which consist of two (2) grease
traps, diversion box, two (2) manual bar screens, two (2) aeration basins with diffused air,
two (2) secondary clarifiers with RAS/WAS lines, one (1) chlorine contact chamber,
dechlorination process, one (1) 22 '/2 inch V-Notch weir with strip chart recorder, and two
(2) sludge holding tanks.
The receiving stream identified as Dixon Branch is classified as Class "C" waters
in the Broad River Basin.
The addition of dechlorination has been applied since the issuance of the last
permit.
OUTFALL 002
This outfall has a permitted flow of 0.005 MGD, which consists of one (1) Oil
and Water separator.
The receiving stream identified as Kings Creek is classified as Class "C" waters
in the Broad River Basin.
There have been no changes since the issuance of the last permit.
SLUDGE MANAGEMENT
PLAN
OUTFALL 001
There has been no change in the sludge disposal method for this facility. When
excessive solids are generated, they are removed by pumping them to our holding tank or
having them pumped out by the septic tank hauler. The septic tank hauler is scheduled
monthly to clean our grease traps and are called out to pump our waste solids as needed.
Wesson Septic Tank Service's disposal site is normally at the Cliffside Sanitary District
in Rutherfordton County North Carolina.
OUTFALL 002
There has been no change in this process to date. This is an Oil and Water
Separator, which does not generate any bio-solids. This process is pumped as needed, by
Wesson Septic Tank Service.

renewal with no changes, or only minor changes such as TRC, NH3,
name/ownership changes). Include conventional WTPs in this group.
This is a MORE COMPLEX EXPEDITED permit renewal (includes
Special Conditions (such as EAA, Wastewater Management Plan), 303(d)
listed, toxicity testing, instream monitoring, compliance concerns, phased
limits). Basin Coordinator to make case -by -case decision.
YES_ This permit CANNOT BE EXPEDITED for one of the following reasons:
• Major Facility (municipal/industrial)
• Minor Municipals with pretreatment program
• Minor Industrials subject to Fed Effluent Guidelines (lb/day limits for BOD, TSS,
etc)
• Limits based on reasonable potential analysis (metals, GW remediation organics)
• Permitted flow > 0.5 MGD (requires full Fact Sheet)
• Permits determined by Basin Coordinator to be outside expedited process
