Dear Mr. Testa:
A letter of request for Authorization to Construct was received July 8, 1986, by
the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the
construction of a 0.0145 MGD wastewater treatment facility consisting of the following
additions to the exising 0.0075 MGD extended aeration treatment plant serving Roadside
Truck Plaza: a new 900 gallon capacity septic tank to remove oil, grease and silt
from wastewater from a new truck wash facility, a flow splitter box, a 10,000 gallon
capacity aeration tank with dual 57 CFM blowers installed in parallel with the
existing plant, an 1,800 gallon capacity settling tank, a new 900 gallon baffled
chlorine contact tank with hypochlorinator to treat the combined effluent from both
plants, a new effluent weir box with recording flow meter, a 900 gallon capacity
aerated sludge holding tank to serve the entire plant, and all associated piping,
valves, fittings and appurtenances, including approximately 1,600 lineal feet of new
3-inch gravity sewer with clean -outs for effluent discharge.
This Authorization to Construct is issued in accordance with Part III paragraph C
of the NPDES Permit No. NC0032867 issued July 1, 1986, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0032867.
The Permittee must employ a certified wastewater operator in accordance with Part
III paragraph D of the referenced permit.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143.215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.

The Asheville Regional Office, telephone number 704-253-3341, shall be notified
at least twenty-four (24) hours in advance of start-up of the installed facilities so
that an in -place inspection can be made prior to operation. Such notification to the
Regional Supervisor shall be made during normal office hours from 8:00 a.m. until 5:00
p.m. on Monday through Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES
permit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.
The wastewater flowrate discharged to and treated by these facilities shall not
exceed 14,500 GPD until such time as NPDES Permit No. 0032867 is modified to allow the
discharge of additional treated wastewater.
