The enclosed application has been completed. A check is enclosed for all facilities. Our facilities are pumped yearly by Lentz Septic Tank Service NCS#00147. This includes any unwanted septage as well as sludge generated by our facilities. I would like to request that influent sampling be waived for Starmount High School. It is my understanding that this is used primarily for industry. It seems to be an unnecessary expense for our school systems. Sincerely, Dr. Barbara R.
