The Division of Water Quality's NPDES Unit has reviewed your request for an Authorization to Construct
(ATC) at the East Bend Elementary WWTP and finds the proposed improvements to be satisfactory and
hereby grants authorization the installation of a 40 gpm UV unit to be used for disinfection. It is the
understanding of the Division that the existing chlorination/dechlorination disinfection system will be
retained on site in case any extended maintenance is required for the UV unit.
This Authorization to Construct is issued in accordance with NPDES Permit No. NC0029611, issued
March 8 2001, and shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the terms and conditions of the permit. This ATC in no way affects
the permitted flow of 0.007 MGD.
The Permittee shall notify the Water Quality Supervisor of the Winston-Salem Regional Office, telephone
number (336) 771-4600, at least forty-eight (48) hours prior to operation of the installed facilities, so that an
in -place inspection can be made. Notification shall be made during the normal office hours of 8:00 a.m. until
5:00 p.m., Monday through Friday.
Engineer's Certification
Upon completion of construction and prior to operation of the modified facility, the Permittee•shall provide
the Division with a certification (copy enclosed) from a professional engineer registered in North Carolina
certifying that the permitted facility has been installed in accordance with the NPDES Permit, this
Authorization to Construct, and the approved plans and specifications. The Certification should be submitted
to: NCDENR / DWQ, NPDES Unit, 1617 Mail Service Center, Raleigh, North Carolina 27699-1617.
Requirements for Certified Operator
The Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC)
of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least
equivalent to or greater than the classification assigned to the wastewater treatment facilities by the
Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate
type and grade to comply with the conditions of Title 15A, Chapter 8G, .0200. The ORC of the facility must

weekends and holidays; and must properly manage and document daily operation and maintenance of the
facility and comply with all other conditions of Title 15A, Chapter 8G, .0200.
Additional Requirements
The Operational Agreement between the Permittee and the Environmental Management Commission is
incorporated herein by reference and is a condition of this Permit. Noncompliance with the terms of the
Operational Agreement shall subject the Permittee to all sanctions provided by G. S. 143-215.6 for violation

The scope of the project includes the installation of an ultra -violet (UV) disinfection system.
RECOMMENDATIONS
Approve installation of a UV disinfection system capable of disinfecting a
peak flow of 200,000 gpd (with one unit out of service)': The system will be
equipped with a UV sensor connected to the existing autodialer to alert the
ORC if the unit is not performing properly.
SUMMARY
Executive Summary
NPDES permit NC0029611 for the subject facility was issued on December 15, 2000. The
treatment works currently consist of:
• Septic tank
• Dosing tank
• Surface sand filter
•Tablet chlorination
The existing permit contains limits for both fecal coliforrn and total residual chlorine. The
installation of the proposed UV system should allow East Bend Elementary WWTP to consistently
meet these limits. The proposed design also conforms with the NPDES Unit's UV guidance.

The UV unit will be cut into the existing effluent force main. The system has been sized to be
compatible with the pumping rate at the plant (peak=10.4 gpm). Additional UV lamps, sleeves and
holders will be kept on site. The system will be interfaced with the existing autodialer such that
the ORC will be alerted in the event of a power loss or when the UV intensity is insufficient to meet
permit limits. In the event of a power outage or other problems with the units, the ORC has the
option of switching over to chlorination/dechlorination to achieve disinfection. Loss of power at
this plant results in the stoppage of flow through the plant (all flow remains in the septic tank).
The proposed unit meets the minimum design requirements for a facility of this size as it is based
on a peaking factor of nearly 10. Spare bulbs and parts shall be kept on site.
Compatibility with existing equipment: The proposed project is compatible with the existing
treatment system and will allow the facility to meet its TRC and fecal coliform limits.
Recommendations
I recommend that the Authorization to Construct be issued for the installation of a UV disinfection
system.
