Hi Charity,
I will need a hard copy submittal with original signature as well as an electronic copy of the updated renewal form and
the renewal submittal package. The electronic submittal requirement can be done by flash drive, floppy disk, or a
scanned submittal to my email address. I do not need an additional processing fee, so on page two Section CA Submittal
Requirements please note that a fee was previously provided.
The letter provided in the previous email is sufficient for the signature authority requirement. Please include this letter
with the hard copy submittal to our Wilmington Regional Office.
The (if applicable) from Number 6 of Section C. is not yet required for submitting the renewal application. It will be in the
future however, currently it is not. If you can provide this letter please do so, however if not submitted it will not effect
the renewal process.
Once I receive the updated application, I will let you know if additional information is needed in order to process the
renewal.

Thanks for letting me know. I am not sure where I got that form, but the information seems to be identical with the
exception of the two items below. So, to be clear, I need to fill out this form with a new notarization and need to
include these two new requirements? The first one will require an inspection from a PE- does that need to be the
original PE that certified the plan?
[IF APPLICABLE] If the project has been built, one original hard copy and one electronic copy of a signed, sealed,
and dated letter from a licensed professional stating that the SCMs have been inspected, and that
they have been built and maintained in accordance with the permit.

copy and one electronic copy of documentation from the NC Secretary of State, or other official documentation,
