We are forwarding Permit No. SW8 970921 dated May 5, 1999, for the construction of W WTP Expansion. This permit shall be effective from the date of issuance until May 5, 2009, and shall be subject to the conditions and limitations as specified therein. Please pay special attention to the Operation and Maintenance requirements in this permit. Failure to establish an adequate system for operation and maintenance of the stormwater management system will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. The stormwater control has been designed to handle the runoff from a 3.5" storm event. 3. The project proposes 55,757 square feet of impervious area. The tract will be limited to the amount of built -upon area indicated on page 3 of this permit, and per approved plans 4. SCHEDULE OF COMPLIANCE The stormwater management system shall be constructed in it's entirety, vegetated and operational for its intended use prior to the construction of any built -upon surface except roads. During construction, erosion shall be kept to a minimum and any eroded areas of the system will be repaired immediately. The permittee shall at all times provide the operation and maintenance necessary to assure the permitted stormwater system functions at optimum efficiency. The approved Operation and Maintenance Plan must be followed in its entirety and maintenance must occur at the scheduled intervals including, but not limited to: a. Semiannual scheduled inspections (every 6 months). b. Sediment removal. C. Mowing and revegetation of side slopes. d. Immediate repair of eroded areas. e. Maintenance of side slopes in accordance with approved plans and specifications. f. Debris removal and unclogging of outlet structure, orifice device and catch basins and piping. g. Access to the outlet structure must be available at all times. 4. Records of maintenance activities must be kept and made available upon request to authorized personnel of DWQ. The records will indicate the date, activity, name of person performing the work and what actions were taken. 5. The facilities shall be constructed as shown on the approved plans. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 6. Upon completion of construction, prior to issuance of a Certificate of Occupancy, and prior to operation of this permitted facility, a certification must be received from an appropriate designer for the system installed certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting documentation. Any deviations from the approved plans and specifications must be noted on the Certification. 7. A copy of the approved plans and specifications shall be maintained on file by the Permittee for a minimum of ten years from the date of the completion of construction. 8. Prior to the sale of any portion of the property, an access/maintenance easement to the stormwater facilities shall be granted in favor of the permittee if access to the stormwater facilities will be restricted by the sale of any portion of the property. 9. 'rhe permittee is responsible for verifying that the proposed built -upon area does not exceed the allowable built -upon area. 10. The runoff from all built -upon area on the project must be directed into the permitted stormwater control system. 11. If the stormwater system was used as an Erosion Control device, it must be restored to design condition prior to operation as a stormwater treatment device, and prior to occupancy of the facility. State Stormwater Management Systems Permit No. SW8 970921 12. The following items will require a modification to the permit: a. Any revision to the approved plans, regardless of size. b. Project name change. C. Transfer of ownership. d. Redesign or addition to the approved amount of built -upon area. e. Further subdivision of the project area. f. Filling in, altering, or piping of any vegetative conveyance shown on the approved plan. In addition, the Director may determine that other revisions to the project should require a modification to the permit. 13. The Director may notify the permittee when the permitted site does not meet one or more of the minimum requirements of the permit. Within the time frame specified in the notice, the permittee shall submit a written time schedule to the Director for modifying the site to meet minimum requirements. The permittee shall provide copies of revised plans and certification in writing to the Director that the changes have been made. III. GENERAL CONDITIONS This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to enforcement action by the Division of Water Quality, in accordance with North Carolina General Statute 143-215.6A to 143- 215.6C. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 4. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement Stormwater management systems. 5. The pernittec grants DENR Staff permission to enter the property for the purpose of inspecting all components of the permitted stormwater management facility. 6. The permit may be modified, revoked and reissued or terminated for cause. The filing of a request for a permit modification, revocation and reissuance or termination does not stay any permit condition. Permit issued this the 5th day of May, 1999. Inspect the infiltration pond(s) for erosion, trash accumulation, grass coverage, and general condition of the facility. 2. Repair eroded areas immediately, re -seed as necessary, keep side slopes and bottom mowed, and remove trash. ' ' 3. Check any inlet piping, swales and catch basins. Repair broken piping, re -seed swales, and remove trash. 4. Inspect the bypass/overflow structure for blockage and deterioration. Remove blockage and repair structure to originally approved design specifications. 5. Check the depth of the pond(s) every six months. If siltation has occurred to the point that only 75% of the original design depth is provided, remove the excess silt to the original depth. It appears that a new road has been added to the plan. Please revise the application to account for this increased impervious area, and revise the calculations to account for the increased runoff. Please label the proposed high side and low side spot elevations for this road. 2. To what side of the road will the runoff be directed? The detail suggests that the runoff will be directed to the outside, but it is not clear how it will be collected and drained into the infiltration pond from there, since no ditches or swales are shown. 3. Please have an engineer seal the plans. 4. Please revise the plan's construction limits to include the new road. 5. Please provide additonal proposed spot elevations for the road. Since the section detail indicates one continuous cross slope, please label the proposed elevations for both the high and low sides of the road at intervals of say 100'. A preliminary review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: I don't know how to make this any clearer. You must show sufficient grading in the form of existing and proposed contours and spot elevations in order to receive this permit. The runoff from all impervious areas of the project must be collected and directed into the stormwater system. The grading as shown is still not sufficient to determine how the runoff from the impervious areas will be directed into the system. Please don't brush this off. This is a state required PERMIT that the applicant as applied for. If you're not serious about taking care of this, please let me know so that I can return the package to the applicant and they can start all over again from the beginning. 2. Does the road have roadside ditches which will collect the runoff? How is the road graded? Does it have a normal crown section, a reverse crown, or a continual slope from one side to the other? By adding elevations to the road, you can show how it is to be graded, and how runoff will be collected and sent to the pond. Does the runoff enter the infiltration basin at a single point, or does it just sort of sheet flow on over that way? Please draw in on the plans how runoff will get into the basin. 4. For all submittals, two (2) sets of plans are required. Subsurface conditions at the site were investigated with four test borings which were drilled at the approximate locations shown in Figure 1. The boring locations had been staked in the field by others prior to the drilling crew's arrival on site. Each boring was drilled to a depth of 30 feet below the existing ground surface, and soil samples were obtained by the standard penetration test method (ASTM D-1586) to'determine the consistency of the subsurface soils. This report presents the findings of our investigation with recommendations for support of the proposed structures based on these findings. We understand that the proposed expansion will include the construction of a Flare Stack, a Cooling Tower, an Anaerobic Reactor, and an Equalisation Tank. The Flare Stack, Cooling Tower, and Anaerobic Reactor will be supported by mat foundations designed for a soil contact stress of 3000 pounds per square foot (psf), whereas the 100-foot diameter Equalization Tank will bear on a concrete ring wall designed for a soil contact stress of 2000 psf. The soil contact stress beneath the 30-foot bigh Equalization Tank will also be about 2000 psf. At Boring No. B-2, though, loose clean sands were noted from a depth of approximately 18 feet to the completed boring depth of 30 feet.. the medium dense to dense sands exhibited standard penetration resistance values ranging from 13 to 40 blows per foot, whereas the loose sands at Boring No. B-2 had penetration resistance values in the 5 to 9 blows per foot range. Groundwater was noted during the drilling operations at depths ranging from approximately 12 to 14 feet below the existing ground surfhce. The site groundwater level will fluctuate with climatic and seasonal changes, however, and may be higher at other times of the year. A generalized subsurface profile, which has been prepared from.the subsurface data to graphically illustrate the subsurface conditions encountered at the test borings, is attached to this report as Figure 2. Variations from the generalized subsurface profile described above and a more detailed description of the subsurface conditions are presented in the attached Test Boring Records. RECOMMENDATIONS _ This report has been prepared in accordance with generally accepted geotechnical engineering practice for specific application to this project. The conclusions and recommendations contained in ibis report.are based upon applicable standards of our practice in this geographic area at the time this report was prepared_ No other warranty, express or implied, is made. _ The analyses and recommendations submitted herein are based, in part, upon the data obtained from the subsurface exploration. On this basis, we recommend delaying permanent connection of the piping until after the proof fMing of the tanks. Site Preoarati�lto . Site preparation should be initiated by stripping the proposed construction area of any roots, organics, or other unsuitable foundation -supporting material- In addition, any underground . utilities located beneath the proposed sttuertues should be relocated. After the required site stripping work has been completed, we recommend that the exposed subgrade sands be proofrolled with a 10-ton vibratory steel -wheeled roller to densify the exposed sands and to locate any areas of soft or otherwise unsuitable surface conditions. Any area which pumps or ruts excessively should be underact and replaced with a granular structural backfill as described below. Where structural fill is required to reach finished grade, we recommend that a dean, slightly silty, or slightly clayey sand having a Unified Soil Classification of SP. SP- SM, or SP SC be used. The fill should be placed in 6 to 8-inch thick lifts and should be compacted to at least 95 percent of the standard Proctor maximum dry density (ASTM D-698); however, the compaction of the upper 12 inches of subgrade should be increased to at least 98 percent of the standard Proctor maximum dry density. To confirm that the specified degree of compaction is being obtained, field density testing should be performed in each fill lift by a soils technician- At those areas where no structural fill is placed on site, we recommend that the upper 12 inches of the in -place subgrade materials be compacted to at least 98 percent of die standard Proctor maximum dry density - If base course stone is placed as structural fill at the location of any of the proposed structures, we recommend that the base coarse stone be placed in loose lifts not exceeding 6 inches and be compacted to at least 98 percent of the modified Proctor maximum dry.density (ASTM D-1557). In order to verify that the soils 0=11111acd in the foundation excavations are similar to those encountered in the soil test borings, we recommend that all footing subgrade surfaces be evaluated by an experienced soils technician or engineer using a dynamic band cone penetrometer prior to concrete piacemetiL The purpose of this testing is to confirm that the foundation subgrade soils consist of suitable bearing material- The foundations should be constructed as soon as possible after any excavation has been completed to prevent degradation of the exposed soils due to inclement weather. The note on the plans says that wetlands are located within the disturbed boundaries. Please delineate these wetlands, as previously requested. 4. The plans submitted DO NOT show the specific drainage area boundary for this infiltration system. Please clearly delineate it by drawing a line on the plans that accurately represents the drainage area to this system.
