I'm working on completing some of my outstanding projects and Andrews Reach is one of them. We only allow 30 days
for receiving the requested information per 15A NCAC 02H.1042(3)(a)(ii) and unfortunately this project has been waiting
on the requested information since February, which is way over our time limit.
If I do not receive a hard copy, single package submittal of a complete 3-page application with original signatures, and
meeting minutes listing yourself as President of the HOA for Andrews Reach by Monday, June 13`^, 2022 1 will have to
return the project application, all supplementary documentation and the initial application fee of $S05.00 will be
forfeited.
If the project is returned and you wish to re -submit later, you will need to resubmit all required items at that time,
including the application fee.

We need hard copies with original signatures sent to our Wilmington Regional Office before any reviewing. Also, the
scanned application was not complete. Section A. project information and Section C. Submittal Requirements appears to
be left blank. This needs to be completed. The last thing I would need to complete the renewal is a copy of the meeting
minutes held by the HOA or a copy of the INC Secretary of State listing you as the HOA president as listed in Section C.7.

Hi Mr. Thomas,
I have attached the additional information request to this email. I understand many
Homeowner Associations' are managed by management companies such as the
Professional Association Management, however the stormwater permit is in the name
Andrews Reach HOA. Only the President or VP can sign the application to renew the
permit, unless the President or VP sign a letter of authorization giving Mr. Sulkey, who's
name is on the current in-house application, signature authority.
I either need a new application submitted listing the correct President or VP in Section B
along with initials in Section C and a new signature, or Mr. Thomas you may write a
letter of authorization allowing Mr. Sulkey to sign on your behalf. In both instances, I
will need official documentation of your position, often submitted to me as meeting
minutes from other HOAs.
I have attached a new application form if you chose to resubmit the application (a new
fee will NOT need to be submitted). Also, when contact information such as presidents
or addresses change, the Division needs to be notified of such changes. I have also
attached a permit update form to this email so you may update the contact information
for our office. This form does need to be notarized, however there is no fee required for
updating the contact information.

The Wilmington Regional Office received and accepted a State Stormwater Management Permit Renewal
Application for the subject project on February 9, 2022. A preliminary in-depth review of that information has
determined that the application is not complete and that additional information is needed. To continue the review,
please provide the following additional information in a single, hard copy submittal package:
15A NCAC 02H.1040(1)(a): The application was signed by Stephen Sulkey; however the provided
documentation from the NC Secretary of State does not identify Mr. Sulkey as a President or Vice -
President of this corporation. In the case of a corporation, the application must be signed by a principal
executive officer of at least the level of vice-president. Please either:
a. Provide official documentation showing that Mr. Sulkey is a President or Vice -President and
authorized to sign for the corporation;
b. Have one of the identified members of the corporation sign the application documents; or
c. The signature by other agents shall be accepted on the application only if accompanied by a letter
of authorization signed by the appropriate authority. Provide a letter of authorization from one of
the identified members of this corporation authorizing Mr. Sulkey to sign on their behalf.
Please remember to confirm that any revised information is presented consistently throughout the application
documents. Please also remember to provide a hard copy of any updated documents per 15A NCAC
0214.1045(3).
The requested information should be received in this Office by March I, 2022, or the application will be returned
as incomplete. If additional time is needed to submit the requested information, please email your extension
request prior to this due date to the email address provided below with ajustification and a date of when the
requested information will be submitted, which shall be no later than 15 days past the original due date. Please
note that only two letters requesting additional information is allowed in the State Stormwater Program for a
total of 30 days to submit the requested additional information. if the it formation is not satisfactorily provided
after either the second request or 30 days, the project will be returned. If the project is returned and you wish to
re -submit later, you will need to resubmit all required items at that time, including the application fee.

State Stormwater Management Permit #SW8 050414 for the subject project expired on August 7, 2020. This is a
reminder that permit renewal applications are due 180 days prior to their expiration per 15A NCAC 2H.1045(3).
North Carolina General Statutes and the Coastal Stormwater rules require that this property be covered under a
stormwater permit. Failure to maintain a permit subjects the owner to assessment of civil penalties. More
information about the Post -Construction state stormwater program can be found on the following website:

Please submit a complete permit renewal form along with a $505.00 fee and other submittal reqquirements within
30 days. The permit renewal form can be found under the Post -Construction section of this site or at:
construction-2. Also, it was noted in the the review that a designer's certiticatlon that the project was constructer
in accordance with the approved plans as required by the permit has not been submitted. Please include this with
your application.
