Please reference the September 16, 2005 Individual Permit application. The application
stated that the 12'x 9' pentuple barrel culvert, carrying Bloomery Swamp, will be
replaced with a 3-span, 205-foot cored slab bridge. The bridge, as shown in the permit
drawings in the original application, is actually afour- span structure.
There will be no changes to impacts to Waters of the ~US.
