functional 18" drain pipe (Pipe #2) and extend the existing ditch from Pipe #2 to just above Pipe #1 Extending the ditch to UT- Middle Fork South Fork New River will result in temporary impacts on the upstream side of Pipe #1 A total of 25 linear feet of temporary impacts will be necessary for the project including pump around operation for culvert installation All temporary impacts will be stabilized following work with erosion control matting and riparian seed mix No concrete will be used during culvert installation No nprap outfall protection will be installed with the replacement pipe, stream gradient is too steep and the streambed is bedrock controlled All temporary fill used in flow diversion will be removed following culvert installation Town of Blowing Rock, Sunset Drive Culvert Replacement Project NWP #3 PCN Application Page 11 of 13 December 22, 2014 4 Jurisdictional Determinations — 4a 1 iave Jurisdictional wetland or stream determinations by the Corps or State been requested or obtained for this property / project (including all prior phases) in the past? ❑ Yes No ❑ Unknown Comments 4b If the Corps made the jurisdictional determination, what type of determination was made? ❑ Preliminary ❑ Final 4c If yes, who delineated the jurisdictional areas?
