Inspection Summary:
Tricia Lowery of the NC Division of Water Resources, Winston-Salem Regional Office conducted a compliance evaluation
inspection of East Bend wastewater collection system (WQCSD0254) on 10/25/2022. Mark Matthews (manager of
operations) was present for the entire inspection. Mr. Matthews did not have logs present for inspector to review and stated
he kept them on his computer at home. Ms. Lowery sent a reminder email to Mr. Matthews on 11/4/2022 concerning
information needed to complete report, but did not receive the information requested as of 11/17/2022. Results of the
inspection are summarized in the following paragraphs:
15A NCAC 02T .0403 Permitting By Regulation
(a)(1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface
waters and to prevent any contravention of groundwater standards or surface water standards.
Observations: Inspector did not have records to review, therefore can not verify if all information is within permit requirements

(a)(2) A map of the collection system shall have been developed and shall be maintained.
Observations: The map is adequate.

(a)(3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule,
spare parts inventory, and overflow response shall have been developed and implemented.

(a)(4) Pump stations that are not connected to telemetry system shall be inspected by the permittee or its representative
every day, 365 days per year, unless the permittee demonstrates that the daily inspections are not necessary because the
pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection
interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry
system shall be inspected once per week.

(a)(5) High -priority sewers shall be inspected by the permittee or its representative once every six months, and inspections
shall be documented.

(a)(6) A general observation by the permittee or its representative of the entire collection system shall be conducted once
per year.

(a)(7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC
02B .0506(a), and public notice shall be provided as required by G.S. 143-215.1 C.
Observations: There have been no SSO's within the last 3 years.

(a)(8)(A) Grease Program shall be in place for a publicly owned collection system that will include bi-annual distribution of
grease education materials to users of the collection system (both commercial and residential) and the legal means to
require grease interceptors for new construction and retrofit and if necessary, of grease interceptors at existing
establishments. The plan shall also include legal means for inspections of the grease interceptors, enforcement for violators
and the legal means to control grease entering the system from other public and private satellite collection systems.

Comment: Inspector did not receive an email from Mark Matthew for FOG information as he did not have it
during the inspection. Only 6 residents are connected to collection system. Inspector was
unable to verify any information concerning FOG and SUO.

Comment: Inspector did not receive an email from Mr. Matthews for spill repsonse plan. Did not have in

Comment: Only one pump station in the system connected to telemetry. It is inspected monthly. Inspector
informed Mr. Matthews pump stations on telemetry must be inspected weekly. There were no
logs for inspector to review as Mr. Matthews stated information was on his computer. Inspector
can not verify if all information is within permit requirements.

Comment: Overgrowth of vegetation at pump station. Mr. Matthews had to tear away at vegetation in
order to open gate to pump station. No pump station inspection log at pump station. Audible
alarm is operable, however visual alarm was inoperable. Mr. Matthews stated he would fix
asap. Inspector told Mr. Matthews to trim down the overgrowth. Inspector did not receive an
email from Mr. Matthews concerning O&M logs Inspector can not verify if all information is
within permit requirements..
