Project Description — This project is a low impact bridge replacement project and is located to the
south and west of Lexington. The area is rural in nature and the surrounding terrain is rolling hills. The
existing bridge on Swicegood Road over First Potts Creek was built in 1972 and is 90' long and 30.3'
wide. The bridge is both structurally deficient and functionally obsolete and has a sufficiency rating of
31.85 out of 100. It is constructed of timber components and precast, prestressed concrete channels.
The proposed bridge will be 100' long and 36' wide. It will be a precast prestressed concrete box beam
bridge on a concrete substructure. It will fully span the creek and will not create any impacts to the
natural flow of First Potts Creek. The approach roadway on each end of the bridge will be updated to
current design standards and guardrail will be placed on each corner of the new bridge where currently
there isn't any guardrail. The existing roadway will be widened only slightly to tie into the new bridge

width. The paved roadway width will match the existing width of 24'. There will beno deck drains placed
over the stream mn this bridge; they will be over the floodp|ainarea. The remaining surface water will
drain off the bridge to the north end where the water will be captured intwo concrete drainage structure,
The water will then flow out through a 15° drainage pipe. There will boa rip rap energy dimsipotoratthe
outlet of the pipe. The outlet of the pipe will be approximately G5'hnm the edge ofthe stream. The outlet
area io flat and well vegetated. All water from the roadway will flow through vegetation prior tmentering
the stream; there will be no direct discharge into the stream where currently water from the bridge flows
directly into the stream through the open bridge rail. There will be approximately S5 linear feet Vfimpacts
10 First Potts Greek ho the southeast side of the bridge, This accounts for 1O linear feet ofpermanent
impacts due to bank stabilization and 85 linear feet of temporary impacts due to the placement of an
impervious dike for dewatering. There will be approximately 205 linear feet of impacts to the UT to First
Putts Creek that flows in the roadside ditch om the southeast side of the bridge. This accounts for 145
linear feet cf permanent impacts due to realignment of the channel and 60 linear feet of temporary
impacts due boa temporary stream crossing and dmvvateMng. All nf the impacts on this project are caused
by the widening of the shoulder inthis area to accommodate the placement of the guardrail at the bridge
approach. Impacts were minimized by limiting widening of the roadway to only the areas necessary to
accommodate guardrail and only installing the minimum amount of guardrail, 75 linear feet, required by
the sub-regional tier design guidelines. Also, the fill slope was designed at 2:1 in this area. The existing
fill slope is 1:1 or steeper in this area and therefore didn't allow for any widening and was creating safety
concerns with the shoulder washing. The UT|s being shifted bm the east toallow for the widening mfthe
shoulder. Due hm the slope of the channel (between 2 and 5.58%),class B rip rap is being used asmliner
ho prevent scour. No mitigation is required for project impacts. Please reference email dated November
18.2018stating that the US Army Corps of Engineers would not require mitigation. Impacts are below

I concur with your review in that the minor road widening is associated with the bridge replacement and
not a widening for a road upgrade. Therefore, the project is verified under the low impact authorization.

Sorry to bother you about this project again. In Stephanie's original email she stated that there was
roadwork which led to the need for the guardrail being moved, and hence the stream needing to be
relocated. Technically the bridge is being widened and in order to match the existing road with the
wider bridge, the road will be tapered by less than 100'. This will result in the guardrail being shifted
out. Is it possible to do this under a NW3 instead of a NW14? This would allow us to do the work
under the Low Impact Program and utilize the PCE from Federal Highways in order to get Right of
Way. If not, we'll have to delay the project. Thanks!

This is in reference to a bridge replacement project in Davidson County on Swicegood Road. You were
in previous communications with Stephanie regarding impacts to a perennial UT to South Potts Creek
that will need to be relocated for the addition of guardrail and fill slope. The impacts to the UT have
been adjusted and should be less than 100'. The stream will be relocated and lined (both bed and
bank) with rip rap. Stream slopes are 3 -5 %. I would like to know if you will require mitigation on the
channel, and if so at what ratio. The existing perennial stream is ditched at the road fill in the project
area. I've attached a copy of the Stream Quality form. Let me know if you need anything else.

as well as bridge replacement; so, submit a pcn for a nwp 14 for this one.

We have a low impact bridge that will have greater than 150 feet of linear stream impacts due to
guardrail. I went to the project this past Tuesday with Matt Jones and it is a stream.
The road will be widened to meet new standards and DOT has to place fill in the stream to allow for the
guardrail.
Shifting the right of way to the other side of the road will create a curve that will lengthen the project
limits and also create a reverse curve on each end of the project.
Changing the slope would not solve the problem because the guardrail would still impact the stream. I
have a few questions based on what I just mentioned. Would this bridge still be allowed on the low
impact program with 150 of linear stream impacts? What kind of mitigation would be required and also
other than reporting the 150 feet of impacts on the attachment G is there anything else needed on that
form?
The bridge is located in Davidson County on Swicegood Road. It crosses First Potts Creek which has a
classification of C and is not 303(d).

I also have another one on Jackson Hill Road in Davidson County over Cabin Creek class WS IV, not
listed as 303(d). We have a temporary easement to put in a driveway to give a farmer access to his
field. The driveway will impact approximately 0.02 acres of wetlands. What is required other than noting
the impact on the Attachment G and do we need to submit anything else?
