NCDEQ Comment: In response to Comment C.2 of the Additional Information Request dated August 12, 2022, a flow reduction document from the NC Department of Health and Human Services (NCDHHS) was provided. Unfortunately, NCDHHS Flow Reductions are only valid for below -grade septic systems permitted under 15A NCAC 18A. The provided approval does not satisfy the requirements of 15A NCAC 02T .0114 that applies to Non -Discharge systems permitted under 15A NCAC 02T. Please contact Mr. Charlie Miller with NPDES Municipal Permitting (charlie.miller@ncdenr.gov) for information regarding the appropriate flow reduction approval process. M&C Response: Please find attached the revised Permit Application - Section IV. General Requirements Item 7. A2. NCDEQ Comment: The response from Applied Resource Management to Comment E.1 indicated that the unsuitable A horizon soils are to be undercut which corresponds to an excavation depth of approximately 12-14 inches below grade. Upon removal appropriate coarse -grained sand fill material is to be incorporated into the Bh horizon and backfilled to the existing grade. An additional layer of appropriate coarse -grained sand fill material is to be added to an elevation approximately 24 inches above the existing soil surface to allow for positive drainage at the site and to satisfy the requirements for the Seasonal High Water Table (SHWT). The soil scientist identified appropriate coarse -grained sand fill material as a C-33 or 2S concrete sand backfill material having a saturated conductivity equal to or greater than the Bh horizons measured in situ. Unfortunately, this design requirement was not reflected in Sheet C 103 of the Engineering Plans and Section 12000 of the Specifications and is a key aspect of the proposed design. Please revise the engineering documents to reflect the recommendations of the soil scientist. M&C Response: Sheet C103 has been revised to reflect the recommendations of the soil scientist. Approximately 12-14 inches of the A horizon will be removed below the existing grade. C33 or 2S concrete sand is to be incorporated into the Bh horizon to a depth of approximately 4-6 inches and is to be backfilled to the existing grade. Additional C33 or 2S concrete sand fill shall be placed to a depth of approximately 24 inches above the existing grade. This area will be backfilled with coarse - grained sand with a C-33 or 2S concrete sand back to the existing grade. Note 4 of the Irrigation Field Notes on sheet GOl has been revised to reflect this design as has Section 12000 of the Specifications. Ft. NCDEQ Comment: See Comment A.2 regarding the Sheet C 103 profile view of the irrigation area and proposed fill placement. M&C Response: Refer to M&C Response to Comment A.2. F2. NCDEQ Comment: Revised Plan Sheet M100 does not depict a screened vent or proper emergency contact signage for the pump tanks. Please revise the plans to include these items as either a detail drawing, note, or call -out. M&C Response: Sheet M100 has been revised to depict a screened vent for each tank. A call -out and a note is included on the sheet that the control panel shall include information pump tank shall be equipped with the proper emergency signage is included in the notes for the 1,500 gallon storage tank. GI. NCDEQ Comment: See Comment A.2 regarding the lack of specifications for the removal of A horizon soils within the drip irrigation mound footprint described in Section 12000. M&C Response: Section 12000 of the Specifications has been revised to reflect the recommendations of the soil scientist. The revised section is included in this submittal. HI. NCDEQ Comment: The provided calculations for the pollutant loading and removal were completed for a design flow of 208 gallons per day (GPD). The proposed facility with a flow reduction approval will have a permitted flow of 260 GPD. Please revise the pollutant loading and removal calculations to correspond to the requested permitted flow (currently of 260 GPD). M&C Response: The pollutant loading and removal calculations have been revised to correspond to the requested permitted flow (260 gpd). The revised calculations are provided in this submittal. H2. NCDEQ Comment: The pollutant loading and removal calculations included two notes regarding the need for a denitrification return line to the primary tank and a sodium aluminate chemical feed system. These were not depicted on the Engineering Plans, nor were they mentioned within the Specifications. Please provide additional information regarding these treatment components and revise the engineering documents, as needed. M&C Response: The notes have been revised on the pollutant loading and removal calculations so that operational field testing is recommended to determine if carbon feed alkalinity and pH adjustments are required for proper operation. The revised calculations are included as an attachment to this submittal. Kl. NCDEQ Comment: The statement "Report any line breaks or spills to the proper authorities" does not satisfy the requirements of 15A NCAC 02T .0507(a)(4). Please provide spill control provisions that include: response to upsets and bypasses, including control, containment, and remediation; and contact information for plant personnel, emergency responders, and regulatory agencies. M&C Response: The statement "Report any line breaks or spills to the proper authorities" has been removed from the Operation and Maintenance plan. L1. NCDEQ Comment: The provided Residuals Management Plan does not meet the requirements of 15A NCAC 02T .0508(a). Please address the following: a. Please provide a detailed explanation as to how the residuals will be collected, handled, processed, and stored prior to disposal, and include the evaluation of the residuals storage requirements for the treatment facility based upon the maximum anticipated residuals production rate to meet the requirements of 15A NCAC 02T .0508(a)(1) and .0508(a)(2). b. The provided language indicates that the generated residuals will be disposed of using a licensed sludge handler; however, a written commitment that satisfies 15A NCAC 02T.0508(a)(3) was not provided. Please provide a written commitment to the Permittee of a Department -approved residuals management program accepting the residuals that demonstrates that the approved program has adequate capacity to accept the residuals. M&C Response: The Residuals Management Plan has been revised to include language indicating that residuals will be stored within the tankage of the system and will be disposed of as needed to maintain operational needs and requirements but shall be at a minimum removed once every three years. A written commitment from Lewis Farms, a licensed sludge hauler, is included as an attachment to this submittal. Mt. NCDEQ Comment: The letter provided from the North Carolina Natural Heritage Program dated October 10, 2022, identified a threatened species, Mabee's Salamander (Ambystoma mabeei), within the one -mile radius of the project area. Pursuant to 15A NCAC 02T .0105(i), Non -Discharge permits shall provide specific conditions to address the protection of threatened or endangered aquatic species, as provided in plans developed pursuant to 15A NCAC 02B .0110. Please determine whether the construction and operation of the facility directly impacts the identified aquatic species and identify site - specific strategies to maintain or recover the water quality conditions to meet the requirements of 15A NCAC 02B .0110, if necessary. M&C Response: Construction will not directly impact the threatened species, Mabee's Salamander (Ambystoma mabeea), as the project is pursuant to the 15A NCAC 02T rules and therefore minimizes impact from the project by adhering to water quality standards. Contact Applied Resource Management for an inspection of fill after mound has been constructed. 1.03 FINAL LANDSCAPING OF FILL SYSTEM A. The fill must be shaped to shed surface water and shall be stabilized with grass or other suitable cover to prevent erosion. B. The proposed crop type to be planted, established, and maintained on the wastewater irrigation fields is Hybrid Bermudagrass overseeded with Rescuegrass (Hay). Description The Family Dollar WWTF is designed to serve the wastewater treatment and disposal needs for the Family Dollar convenient store in Currie, NC. The design capacity of the WWTF is 260 GPD The plant is designed to provide effluent quality suitable for drip irrigation disposal in accordance with the NCDEQ Non -Discharge Wastewater Irrigation Systems permit. The proposed WWTF includes a 1,000-gallon precast septic tank with effluent filter, 1,300 gallon recirculation tank with two parallel AX20 textile filter treatment systems, ultraviolet disinfection, 1,500 gallon precast storage tank with effluent pump station, and drip irrigation disposal. General Operational Requirements The Family Dollar WWTF will maintain certified wastewater treatment system operators to operate, monitor, and maintain the treatment process and equipment. General operational responsibilities include the following: 1. Comply with NCDEQ rules and regulations as defined in the permit. 2. Monitor, operate, and maintain the treatment facilities. 3. Maintain required records of treatment operations, maintenance, residuals disposal, chemical usage, etc. 4. Perform required compliance testing in accordance with permit requirements. 5. Submit monitoring and other reports as required by NCDEQ. 6. Implement operational procedures for dealing with emergency situations. 7. Comply with safety rules and regulations. 8. Response to Upsets including control, containments and remediation- Audible and Visual alarms shall be provided for the WWTF. 9. Report any line breaks or spills to the proper authorities. Emergency contact information will be posted on signage at the control panel. This contact information will include information for Plant Personnel, Emergency Responders and Regulatory Agencies — phone numbers for all plant personnel, emergency responders and regulatory agencies will be provided on signage. General Operation & Maintenance Requirements and Plan Maintenance of the wastewater treatment and disposal system generally includes the following: 1. Provide preventive maintenance of equipment as required per manufacturer's instructions including valves and valve actuators, pumps and motors, chemical feed systems, instrumentation and control, samplers, etc. 2. Prepare a preventive maintenance schedule, keep maintenance records updated. 3. Maintain necessary tools and supplies on -site for proper maintenance work. 4. Perform field sampling, testing, and process control activities required to optimize the performance of the facility. 5. Repair any broken equipment or piping leaks as required. 6. Keep spare parts, piping repair equipment, etc. in inventory for emergency use and repairs. 7. Provide for general housekeeping and grounds maintenance. Residuals Management Plan 1. Residuals will be stored within the tankage of the system and will be disposed of as needed to maintain operational needs and requirements but shall be at a minimum removed once every three years. 2. Residuals will be disposed of via contract with licensed sludge hauler Lewis Farms. Power Reliability Plan The WWTF is designed with sufficient storage capacity with no existing potential for overflow. The storage tank has the capacity for 1,500 gallons. At 260 gpd, this would provide 5.77 days of storage (1,500 gallons/260 gallons/day). Additionally, there will be a hook-up for a portable back-up generator that can be obtained during instances in which a generator will be needed. Lewis Farms will accept residuals from the Family Dollar - Currie WWTF subject to analysis and pre -acceptance by Lewis Farms policies. Upon approval, the residuals will be managed at the Lewis Farms Residuals Land Application Facility and land applied under their permit.
