Explanation Submitted Notice of Termination Request on 11/16. Received email from Paul Clark
right after submitting stating that Jeremy Westbrook had to be listed as the legally
responsible individual. We had already mailed out the signed NOT Certification
Form, so please disregard that one and we'll mail another one with the correct
information! Sorry for the confusion.
