NC0004952. Please make the necessary corrections to BIMS. If additional information is needed or if the requested
changes should be accompanied by certain forms, then please contact Ms. Carter directly and let her know. The following
changes are needed:
1. Facility name
2. Owner name
3. Affiliations updates
PEM —
With respect to the Owner affiliation type, we have to reflect in our system a person ... this cannot be the organization
name itself, it must be a person. In the case of corporations, the person indicated should be "a principal executive officer
of at least the level of vice-president, or his duly authorized representative, if such representative is responsible for the
overall operation of the facility from which the discharge ... originates" [T15A: 02H .0106(e)(1)]. As a result, please
provide a person's name to replace Steve Olp as the owner affiliation type for CNA Holdings, LLC. Also, did you want a
replacement for Charles Thomas entered into our system as the Facility Contact Person? If so, please provide that
information to Wren. Lastly, in our system affiliations can be associated with a permit number at 3 different
levels: Owner, Facility and/or Permit ... these are reflected on the Affiliations tab as Entity. Please note we do not have
the ability to change the Entity from Owner to Site Contact. If you prefer, it can be changed to the Facility &/or Permit
level. However, I recommend you speak with Mr. Charles Weaver with our permitting group in order to understand the
different levels better. I've copied him on this email ... or you can contact him at 919-807-6391.

Below is a screen shot from the Facility User Management Guide available on the eDMR website. The user group and
privileges table shows the different eDMR user groups (Owners, Facility Administrators, Submitters, etc.) and the
permissions/privileges associated with each group. As discussed, only the Division can assign the Submitter user group to
users. Once the Owner and/or Facility Administrator group has been assigned to an individual within the facility
organization, then that person has the ability to manage users and assign other groups, excluding the Submitter group, to
other members of the facility's organization. Note, if the person assigned the Submitter group is to also perform data
entry tasks, then he/she will also need to be assigned the Data Entry group. The table shows the different privileges
associated with each user group.

A facility Owner and / or Facility Administrator can assign the facility users to the appropriate
user groups. A user can be assigned to multiple groups. Users can only see the menu options
for which that user has privileges.
