issued the E&SC plan approval (DEQ-DEMLR-Mooresville Regional Office). Please download (from eNOT), print out,
complete, scan and upload the NOT Certification Form. On this form, Anita Thomas is the legally responsible individual
and her name goes on the legally responsible person line of the NOT Cert Form. She can authorize another person to
wet sign the NOT Cert Form, but that second person then prints name and title on bottom line of NOT Cert Form. This
also allows second person to digitally sign the eNOT. thx.
