A letter of request for an Authorization to Construct was received October 30,
1987, by the Division and final plans and specifications for the subject facilities
have been reviewed and found to be satisfactory. Authorization is hereby granted for
the construction of. a..027 MGD wastewater treatment:facility consisting ofan influent
surge tank with bar screen, a 40,000 gallon aeration tank, a 5,550 gallon clarifier,
an aerated sludge holding tank, a hypochlorinator, chlorine contact tank, a 1,000
gallon post aeration tank, two (2) 250 CFM blowers, -effluent flow measurement and
stand-by power.
This Authorization to Construct is issued in accordance with Part III paragraph C
of the NPDES Permit No. NC0065358 issued May 22, 1986, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0065358.
The Permittee must employ a certified wastewater operator in accordance with Part
I paragraph D of the referenced permit.
The sludge wastes generated from these treatment. facilities must be disposed of
in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
In event the facilities fail to perform satisfactorily in meeting its NPDES
,,ermit effluent limits, the Permittee shall take such immediate corrective action as
may be required by this Division, including the construction of additional wastewater
treatment and disposal facilities.

Upon completion of construction and prior to operation of this permitted
facility, a certification must be received from a professional engineer certifying
that the permitted facility has been installed in accordance with with the approved
plans and specifications.
