On March 2, 2011, Autumn Romanski, inspector from the Raleigh Regional Office visited your single-
family residence (SFR) wastewater treatment system to evaluate compliance with the above permit to
discharge wastewater. Your assistance over the phone with regards to the inspection was greatly
appreciated. The checked boxes below show what conditions were noted at your facility:
® In compliance. You are reminded to regularly maintain the chlorine disinfection, have
the effluent sampled once a year, and have the septic tank pumped out every 3 to 5 years.
You are reminded disinfection with chlorine tablets is required. The permittee was in
compliance with the requirement. You, the permittee, are responsible for always having chlorine
tablets in place. They must be the kind for wastewater treatment and not for swimming pools.
Pumping the septic tank. You are to have the septic tank pumped out every 3 to 5 years. A
pumping company can check the status periodically and determine when pumping is required.
The permittee reported the septic tank was pumped in the last 3-5 years.
E Failure to analyze the effluent from your system once each year. See Part I(A) of your
permit about his requirement. Attached is a list of laboratories in NC certified to provide this
service. Make arrangements for sampling to be carried out within the next 3 months, as flow
allows, and submit results to this office within 3 weeks after the sampling has been done.
® Locations of treatment units are unknown. Please become familiar with the location of
the sand filter, (as our records indicate and inspector observations noted) the sand filter is
located in the backyard down slope of the septic tank.
® Other: Please submit the enclosed Permit Renewal Form to DWQ as soon as possible.

The permittee was compliant with the permit requirements with the exception of sampling the discharge. No
sewage issues were observed and the discharge was clear, with no solids or sewage smell, the day of the
inspection.
