Dear Mr. Smith:
The Division of Water Quality (the Division or DWQ) hereby issues the attached NPDES permit
for the subject facility. We issue this permit pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated October 15, 2007, or as subsequently amended.
Changes to Your Previous Permit. The Division understands that you have made no significant
alterations to your facilities since last renewal. We have therefore made minimal updates to your
facility map and treatment -component description. We have added parameter codes to the effluent -
monitoring page for your convenience [see Supplement to Cover Sheet and Section A. (1.)].
Considering that you have no need to disinfect your effluent, we have deleted Total Residual
Chlorine (TRC): We have also deleted Naphthalene monitoring at your request based on your
analyses indicating "not detected" over this past permit cycle (database n = 48).
Concerning BTEX. Please note that we may not comply with your request to delete monitoring for
Benzene, Toluene, Ethylbenzene, and Xylene (BTEX) because these databases are insufficient in
size to support such action (n = 7). The Division requires a minimum of 12 sample events before
running a defensible Reasonable Potential Analysis (RPA). However we have added a footnote
providing you the option to petition the Division for future data review [see Section A. (1.),
footnote 3]. Finally, we are concerned with discrepancies in your reported units -of -measure with
regard to BTEX (mg/L vs. µg/L). Please be advised that your permit requires you to employ the
most sensitive approved test method practically and technologically available (see permit Boiler
Plate, Sect D. Part 4, ¶ 3) considering a given parameter's instream water -quality standard. For
example, Benzene requires all testing to a practical quantitation level (PQL) of 1 µg/L.

If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within
thirty (30) days after receiving this letter. Your request must take the form of a written petition
conforming to Chapter 150B of the North Carolina General Statutes, and you must file it with the
office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
Unless such a demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division
may require permit modification, or revocation and re -issuance. Please note that this permit does
not affect your legal obligation to obtain other permits required by the Division of Water Quality,
the Division of Land Resources, the Coastal Area Management Act, or other federal or local
governments.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions described herein.
Duke Energy Carolinas, LLC

1. continue to operate an existing Surface -Water Pollution Prevention System to treat stormwater, wash -
down water, calibration water, and hydrostatic tank -testing water and similar wastewaters in proximity
to above -ground storage tanks (ASTs) and appurtenant piping where total surface bulk -storage of
petroleum hydrocarbon fuels exceeds one (1) million gallons, utilizing

• polishing filter
• discharge line with hand -operated valve (normally closed)
located at Rockingham County Combustion Turbine Station, 240 Earnest Drive, Reidsville,
Rockingham County, and
2. discharge from said treatment works via Outfal1001, at a location specified on the attached map, into
an unnamed tributary to Little Jacobs Creek [stream segment 22-32-2], currently a Class C waterbody
within Subbasin 03-02-02 of the Roanoke River Basin.

Monitoring Report (DMR) indicating "No discharge." Flow shall be monitored with each
discharge event and may be monitored in one of four ways:
• measure flow continuously
• calculate flow based on total rainfall per area draining to the outfall
• estimate flow at 20-minute intervals during the entire discharge event, or
• report flow based on discharge pump logs.

3. Following hydrostatic testing, the Permittee shall discharge no tank (or pipe) contents
unless discharge analyses show Benzene concentration below 1.19µg/L and Toluene
concentration belowl l µg/L.

A. (2.) ACUTE TOXICITY MONITORING (ANNUAL)
The Permittee shall conduct annual toxicity tests using protocols defined as definitive in EPA Document
EPA/600/4-90/027 entitled "Methods for Measuring the Acute Toxicity of Effluents to Freshwater and
Marine Organisms." The monitoring shall be performed as a Fathead Minnow (Pimephales promelas)
24-hour static test. Effluent samples for self -monitoring purposes must be obtained below all waste
treatment. The Permittee '11 conduct one test annually, with the annual period beginning in January of
the calendar year of the e ' - ctive date of the permit. The annual test requirement must be performed and
reported by June 30. If no , 'scharge occurs by June 30, notification will be made to the Division by this
date. Toxicity testing will be a erformed on the next discharge event for the annual test requirement.
The parameter code for this test 's TAE6C. All toxicity testing results, required as part of this permit
condition, will be entered on the ffluent Discharge Form (MR-1) for the month in which it was
performed, using the appropriate p: ameter code. Additionally, DWQ Form AT-1 (original) is to be sent

Test data shall be complete and accurate and ' elude all supporting chemical/physical measurements
performed in association with the toxicity test , as well as all dose/response data. Total residual chlorine

Should any test data from either these monitoring equirements or tests performed by the North Carolina
Division of Water Quality indicate potential impac , to the receiving stream, this permit may be re -opened

Comments:
Correspondence with the Permittee — the renewal application requests the Division to remove monitoring for
BTEX. However, the databases for these parameters contain only seven (7) samples each, and are judged
insufficient to run a defensible RPA. Moreover, a significant percentage of the data submitted were reported in
mg/L instead of µg/L (see attached), a test -sensitivity inappropriate to the relative stream standard for each
parameter. This facility discharges under zero -flow receiving -stream conditions allowing no credit for dilution
in the environment.
Proposed Renewal - Changes from Previous Permit — Because this facility does not generate domestic waste,
and because it has no reason to disinfect its effluent, DWQ will remove TRC monitoring and limit from this
permit for renewal. We have also deleted Naphthalene monitoring at the Permittee's request based on analyses
indicating "not detected" over this past permit cycle (database n = 48).
Toxicity — The Permittee passed all quarterly Whole Effluent Toxicity (WET) tests since Jan2006 (see
attached).
Compliance - During the previous permit cycle, this facility received several Notices of Violation (NOVs) but
for "monitoring frequency" only; none of these proceeded to civil -penalty assessment (see attached).

7. Describe the treatment system List all installed waste treatment components with capacities,
describe the processes that generate wastewaters. If the space provided is not sufficient attach a
separate sheet of paper with the system description.
The wastewater system is primarily for the treatment of storm water, process water, system
blow downs, wash downs, air conditioning condensate and other discharges associated with
the operation of the gas turbines. The treatment system is designed to remove oil/ fuel from
the wastewater. The treatment system consists of an oil water separator (OWS) with a
coalescing filter that functions as a polishing filter. Wastewater is collected in the OWS sump,
passes through the OWS and then is pumped through the polishing filter before discharge to
the 001 outfall.
The OWS and coalescing filter have a capacity of 11,600 gallons. The discharges are primarily
influenced by precipitation and plant operations, which can vary greatly seasonally. Design,
average, and maximum instantaneous discharge flow rates from the OWS and 001 outfall
range between 100 and 200 gpm respectively. Discharge duration varies from 30 - 60
minutes.
The oil and solids collected by the OWS are pumped out by a contractor and taken off site for
recycling and disposal as needed. These cleanouts are infrequent, with the last cleaning being
completed in July of 2010.

Briefly describe the manufacturing process(es):
The plant consists of five simple cycle gas turbine generator units producing approximately 170
megawatts each for a total generating capacity of approximately 850 megawatts. The primary fuel
consumed is natural gas, with #2 diesel fuel as a secondary fuel. Raw water is processed through
portable demineralizer cells and stored in a 1.7 million gallon tank.
During the operation of the units, demineralized water is injected into the units for NOx control.
The plant is dispatched for peaking and back-up power to support the regulated Duke Energy system.
