Dear Mr. Lowry:
Division personnel have reviewed and approved your application for minor modification of the subject
permit. Accordingly we are forwarding the attached modified permit page. Please remove the existing
"Effluent Limitations and Monitoring Requirements" page and replace it with the one attached to this
letter.
This modification reduces the effluent monitoring frequencies for Biochemical Oxygen Demand
(BOD5), Total Suspended Solids (TSS), Ammonia Nitrogen (NH3-N) and Fecal Coliform from 3/week
to 2/week monitoring. The modification was based upon an evaluation of three years of facility effluent
monitoring data. The facility's performance satisfies the criteria established in the "DWQ Guidance
Regarding the Reduction of Monitoring Frequencies in NPDES Permits for Exceptionally Performing
Facilities" as approved by the Director of the Division of Water Resources on October 22, 2012, and
justifies reduced monitoring for these parameters. The modified monitoring frequencies will become
effective on April 1, 2014.
This modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1
and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Resources or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

2. The monthly average effluent BOD5 and Total Suspended Solids concentrations shall not exceed 15 percent of
the respective monthly average influent value (85% removal).
3. Total Residual Chlorine compliance is required for chlorine or chlorine derivative used for disinfection. The
Division shall consider all effluent TRC values reported below 50 ug/t to be in compliance with the permit.
However, the Permittee shall continue to record and submit all values reported by a North Carolina certified
laboratory (including field certified), even if these values fall below 50 ug/l.
4. Cadmium measurement detection minimum of 1 pg/L.
5. Chronic Toxicity (Ceriodaphnia) P/F at 1.7%; January, April, July, and October. See Special Condition A.(2.) of
this permit. Quarterly sampling shall be conducted at the same time as metal and other effluent parameters
sampling is conducted.
6. See Condition A. (3.) of this permit.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

The Town was unaware that a second species toxicity test was required for the permit renewal.
Once we were told of this requirement, we made arrangements to conduct the tests. One second
species test has been concluded and the other three are scheduled to be concluded prior to permit
expiration.
The Town would like to request that the following modifications be made to the permit:
We request that monitoring for BOD, TSS, Ammonia Nitrogen and Fecal Coliform be
reduced under the "exceptionally performing facilities" criteria. The attached data
(summarized in the following table) indicates that the WWTP effluent has exceeded the
minimum criteria for reduced monitoring. In addition to the monitoring results, the plant
meets the other listed criteria.

It is requested that upstream/downstream monitoring be eliminated. We feel that
monitoring the stream for temperature and dissolved oxygen provides no useful data
regarding the WWTP impacts upon the receiving waters. Permit limitations are issued to
protect water quality, including a minimum effluent dissolved oxygen. "Outside"
influences such as stormwater runoff and reduced velocity in the swampy areas that are
subject to decay and placid conditions makes taking dissolved oxygen levels and
temperatures inconclusive in determining wastewater impacts
If second species monitoring is to be continued to be required for permit renewal, we
request that one per year be included in the permit.
