The Design Build Team has indicated that a plan revision is necessary to ditches on the left side of the road to
increase the amount of excavation on the project to offset fill that the project is adding below the flood pool
elevation. This plan revision results in a wider cross section of the ditches as they tie into the east and west
banks of Little Island Creek. This wider cross section of ditch on the west bank results in an increase of four
linear feet of permanent rip rap impacts along the west bank and an increase of three linear feet of permanent
rip rap impacts along the east bank.

I have attached revised stream impact summary sheet and revised stream permit drawing. This project was
previously permitted using Nationwide Permit Number 3 and WQC Certificate Number 3883. Can this slight
increase in permanent stream impact be approved by submittal of this information to your offices? Or, is a
formal permit modification application required?
