Re: Notice of Violation & Intent to Assess Civil Penalty
We received this notice on 10-21-2022 concerning a review of the May 2022 Non -
Discharge Analysis Report (NDAR1) for our facility. The notice stated that Field site #2
was missing. We have been reporting both fields (#1 and #2) combined in our report
under (Field #1) from the beginning of the operation of this system (15 + Years). We had
always mentioned this item during our annual inspections and the inspectors said that
they saw no problem with that type of reporting. We have now updated the reporting
form to include two fields and you will see that correction in the September Report. We
hope this will take care of the concerns found during the review of the May report. Please
let us know if we need to make any additional adjustments to our reporting. I am
including a blank copy of the revised form.
