I've confirmed that the facility address has already been updated in our system. However, many of the affiliations had
the old physical address. I've updated those just now. Please run a permit summary report on Monday (the database
will refresh in 24-48 hrs) and check to make sure everything is accurate. If the billing address is still incorrect, please
notify Kieu Tran, and she can update the physical address for that as well.

Good Morning,
Our physical location has not changed, however the town changed the name of the road in front of our facility (for only a
short portion of the road) about 8 years ago and it has created shipping havoc ever since. We recently had a break in and
law enforcement were dispatched to a building over a 1/4 mile down the street. As a result, we are making the address
change to reflect our correct 911 address. Julius Chambers Ave is still the name of the highway for a brief portion of
NC109 in front of our location, however National St is the name of our private drive that comes into our property. Our
mailing address for postal mail is still the same at PO Box 8, Mount Gilead, NC 27306.
