Enclosed for your review please find attached one original and one copy of a Pump and Haul
Permit Application for the UNC Bingham Facility. A pump and haul permit is needed at the
facility so that repairs can be made to the wastewater system's recirculating tank.
A description of the wastewater system at the UNC Bingham Facility and additional information
on the issue to be addressed, the proposed pump and haul, and proposed remedy are included as
attached Section C of the application.

Division of Water Resources INSTRUCTIONS FOR FORM P&H 01-16 & SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0200, and Division policies. Failure to submit
all required items will necessitate additional processing and review time. For more information, visit the Wastewater Branch's
website or contact the Regional Office serving your county.
General — This permit application applies to all pump and haul activities of wastewater under the authority of the Division on Water
Resources. Permit coverage is required for the system producing the wastewater. This permit does not apply to the transport of
animal waste from an animal waste management systems, not the transport of wastewater residuals or biosolids.
NOTE: Pump and haul permits are not acceptable long term domestic wastewater treatment alternatives and shall only be issued
in cases of environmental emergencies, nuisance conditions, health problems, or for unavoidable delays in construction of system
previously permitted by the Division.
The Applicant shall submit one original and one copy of the application and supporting documentation.

➢ The appropriate application fee is: $810.00 for flows < 10,000 GPD and $1,310.00 for flows > 10,000 GPD
C. Wastewater Pump and Haul (FORM: P&H 01-16) Application:
® Submit the completed and appropriately executed Wastewater Pump and Haul (FORM: P&H 01-16) application. Any
unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or
due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to
correspond to the section and item to which they refer
D. Receiving Facility Letter:
® Submit a letter from the owner/authority of the receiving wastewater treatment facility, as required by 15A NCAC 02T .0204
(b), stating that the pumped and hauled wastewater will be accepted and specifying the volume and type of wastewater that
will be accepted. The letter should be signed by an authorized signing official in accordance with 15A NCAC 02T .0106(b)
E. Contract Hauler Letter:
® Submit a letter from the hauler stating their capability and willingness to perform the pump and haul activity
® Letter shall include: Owner/Company Name, Contact Information, & Hauling Volume of Vehicle(s)
F. Certifications:
® The Applicant's Certification on Page 4 of the application shall be signed in accordance with 15A NCAC 02T .0106(b). Per 15A
NCAC 02T .0106(c), an alternate person may be designated as the signing official if a delegation letter is provided from a
person who meets the criteria in 15A NCAC 02T .0106(b)

Industrial Pump and Haul Activities
Per 15A NCAC 02T .0203(A)(2), Industrial pump and haul activities are deemed permitted provided the following criteria is met:
➢ The facility notifies the appropriate Division regional office in writing advising of the type of operation, type and quantity of
wastewater generated, location, and the receiving wastewater treatment facility. A letter from the facility accepting the
wastewater (type and quantity) specifically agreeing to accept wastewater from the applicant shall be included.
➢ The wastewater does not contain any human waste.
➢ The waste is collected and discharged into a sewer or treatment system designed and permitted to accept the type of
wastewater being pumped and hauled.
➢ The pump and haul activity is not to alleviate a failing wastewater system.
➢ The Division regional office concurs in writing that the activity meets the criteria in this Rule.

Section C
Summary Description of Wastewater Treatment System
The wastewater treatment system provides secondary treatment for a permitted capacity of 3,556
gallons per day (GPD) of domestic wastewater generated from the three existing animal care
buildings at the Bingham Facility. Wastewater is conveyed to the AdvanTex secondary
wastewater treatment system via a gravity sewer collection system. The AdvanTex secondary
wastewater treatment system consists of an 8,000 gallon domestic wastewater septic tank, a 20
gallons per minute (GPM) septic tank effluent pump station, and a 4,000 gallon AdvanTex
recirculation tank with two 55 GPM pumps to recirculate wastewater through the two AdvanTex
textile media wastewater treatment filter pods. Treated effluent from the Advantex system flows
through a chlorine contact tank with sodium hypochlorite injection for disinfection, an ultrasonic
flow monitoring system, and a duplex 80 GPM secondary effluent pump station to pump treated
wastewater to the 1,122,440 gallon clay -lined wet weather storage basin. A 75,843 gallon clay -
lined effluent storage basin is provided at the effluent pump station for secondary storage of
effluent. The wet weather storage basin includes a duplex 150 GPM irrigation pump station to
convey treated effluent to one of four drip irrigation zones for tertiary treatment in the soils via
uptake by the grass, trees, and shrubs in the drip irrigation fields and dispersal into the surficial
aquifer. See Overall Site Plan and Wastewater Treatmemt System drawings attached.
Issue To Be Addressed
On September 26, 2022, Operators at the UNC Bingham Facility observed a water level in the
recirculating tank of the AdvanTex system lower than usual. Water level was below the level of
the floats. Water level in the tank seemed to recover somewhat throughout the day, and it was
speculated that a float had gotten stuck, pumping down the recirculating tank.
On September 27, 2022, water level in the recirculating tank was again below the level of the
floats. Additionally, effluent flow out of the treatment system for the previous 24 hours was
zero. Water meters that measure potable water used in the buildings show 1021 gallons used
during the same 24-hour period. Note that potable water used is typically somewhat higher than
the amount of treatment system effluent.
Further investigation suggests that the clam shell recirculating tank is leaking along the center
seam of the tank.

To further investigate the issues with the recirculating tank and implement repairs, temporary
Pump and Haul is proposed. Vacuum trucks will remove effluent from the septic tank of the
system (see the attached select drawings from the 2014 renovations of the wastewater treatment
system). The general plan is to pump and haul waste twice a week keeping the septic tank as low
as reasonable. There is an alarm in the Septic Tank Effluent Pump Station that is audible, visual,
and calls out to UNC Campus Police.
Proposed Remedy
Following implementation of Pump and Haul, plans will be developed to either repair or replace
the recirculating tank.

SUBJECT: WASTEWATER / PUMP AND HAUL
Dear Mr. Daw:
Recently you contacted the Mason Farm Wastewater Treatment Plant (NIF'WWTP) and requested
authorization to discharge wastewater into the Wastewater Treatment Facilities of Orange Water and
Sewer Authority (OWASA).
The terms and conditions applicable to the use of the septage receiving facility at the Mason Farm WWTP
are:
1. Only wastewater from the UNC Bingham Facility will be permitted for discharge into this facility for
the next 60 days from the acceptance of this authorization letter. If additional days are needed beyond
the timeframe of acceptance, please contact Wil Lawson with OWASA.
2. A routine sample per tanker is to be collected along with a completed Septage Receiving Record form
(see attachment) which identifies the source and volume of the wastewater must be provided to the
Mason Farm WWTP Staff at the time of each wastewater discharge. Authorization for continued use
of this service at the normal sewer rate listed in line item #3 may be revoked if it is determined that
waste characteristic is beginning to mimic normal septic waste which is charged at a rate of
$182.63/thousand gallons. This determination will be made by OWASA.
3. The charges for handling this wastewater will be billed on a monthly basis. These charges are
$8.45/thousand gallons plus a $30 administration fee for each individual discharge at the MFWWTP.
Failure to pay the charges billed for this service will result in revocation of the authorization to
discharge into the OWASA wastewater facilities.

4. Routine use of the septage facility at the Mason Farm Plant is during normal staffing hours at the facility
(Monday -Friday 5:00 A.M.400 P.M.). Any other arrangement must be made in advance.

for the above -referenced services. This proposal is based upon information provided by UNC Bingham.
Shamrock understands that UNC Bingham requires vacuum truck services to pump septic waste from tanks
and unload at Orange Water and Sewer Authority (OWASA).
Shamrock proposes the following prices for this work:
Vacuum Truck w/Operator (portal to portal) $150.00/hour + ES
Shamrock's proposal is conditioned by the following clarifications:
• Prices are valid for 30 days from the date of this proposal.
• On -site services are offered subject to standard conditions:
o Service will be performed on a non -holiday weekday during normal working hours.
o UNC Bingham will provide access to the work area, cooperate with equipment isolation
needs, supply a water source near the work area and approve all work prior to Shamrock's
demobilization.
o Replacing gaskets, bolts or other equipment is not included in the proposed scope of work.
• Transportation services are offered subject to standard accessorial charges and conditions:
o Shamrock's Energy Surcharge (ES) varies monthly with national average energy costs
published by the U.S. Energy Information Administration. The current energy surcharge is
18.5%.

o After-hours pickup requests are subject to advance approval and an additional charge of
$50.00 per hour.
o Container loading is the responsibility of the shipper unless otherwise arranged in advance.
Bulk loading via vacuum tanker is offered as part of the Shamrock service at no additional

o Out -of -route miles traveled outside carrier control will be charged at a rate of $2.25 per
