Woodlake WWTP
Guilford County
Dear Mr. House:
Division staff in the NC DENR Winston-Salem Regional Office have requested a minor modification to
the subject permit. Due to safety and accessibility concerns, the existing downstream sampling location
should be abandoned for one closer to the outfall. This permit modification changes the minimum
downstream sampling distance in section -A. 1. of the permit. No other changes to the permit have been
made.
Please find enclosed the revised effluent page, which should be inserted into your permit. The old page
may then be discarded. All other terms and conditions contained in the original permit remain unchanged
and in full effect. This permit modification is issued under the requirements of North Carolina General
Statutes 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit modification
are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be a written petition conforming to Chapter 150B
of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail Service
Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and
binding.

A. (1.) EFFLUENT LIMITS AND MONITORING REQUIREMENTS
During the period beginning on December 6, 2007, and lasting until September 30, 2011, the permittee is
authorized to discharge treated wastewater from outfall 001. Such discharges shall be limited and monitored by
the permittee as specified below:

Footnotes:
1. U: at least 50 feet upstream from the outfall. D: at least 100 feet downstream from the outfall.
2. TRC limit takes effect on April 1, 2008. Until the limit becomes effective, the permittee shall monitor
TRC (with no effluent limit).
There shall be no discharge of floating solids or visible foam in other than trace amounts.
A. (2.) PERMIT RE -OPENER: SUPPLEMENTARY MONITORING
The Division may, upon written notification to the permittee, require additional effluent
monitoring that it deems necessary to support its water quality protection and restoration efforts
in the receiving stream.
