Dear Mr. Couch:
In accordance with the application for discharge permit received on August 28, 1998 the Division is
forwarding herewith the subject state - NPDES permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated December 6, 1983.
The only modification from the last permit is addition of a quarterly monitoring requirement for the
parameters of total nitrogen and total phosphorus. This requirement has been added for the compilation of
nutrient data prior to the development of a nutrient management strategy for High Rock Lake located in the
Yadkin River Basin. This monitoring requirement has been added to several dischargers within the
subbasins above High Rock Lake.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the office of Administrative Hearings,
Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such a demand is made, this permit
shall be final and binding.
Please take notice that this permit is not transferable. Part II, E.4. addresses the requirements to be
followed in case of change in ownership or control of this discharge. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, Coastal Area Management Act, or any other Federal or Local
governmental permits may be required.

Summary
Traphill Elementary is requesting renewal of its existing NPDES permit. The
receiving stream, Little Sandy Creek, is not on the 303d list for impaired
streams Review of compliance data for 96-97 indicates no problems with
plant. The facility is meeting all limits. Some exceedences for flow in 1997
based on plumbing problems. School appears to be well operated.
Recommend renewal of existing effluent limits with the addition of
quarterly monitoring for TN and TP..

Traphill Elementary School is requesting renewal of existing permit. Discharges into
Little Sandy Creek. This stream is not on the 303d list for impaired streams. Facility is
permitted for 0.004 MGD with limits of:

Review of compliance data indicated that facility appears to be well operated. It has met
all limits consistently. There were some exceedences of the flow limit in 1997, however
comments on the DMRs indicated that there were plumbing leaks within the system.
Upon repair of the leaks, flow limits of 0.004 MGD were back in compliance. School
was under an SOC in 1995 and early 1996, for BOD5, TSS and fecal coliform. SOC
expired in March 1996 and school has since met limits.
Per Yadkin Basin Plan - Will include quarterly monitoring for TN and TP to gather data
for determining a nutrient management strategy for High Rock Lake.
Recommend renewal of the permit with all existing limits. All parameters and
monitoring frequencies remain the same, with the exception of the addition of
quarterly monitoring frequency for TN and TP.
