4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The project site is located along Little Snow Creek in Danbury, NC. The portion of stream to be restored runs through a cow pasture. A stream restoration project was completed on this
reach of stream in early 2017, however several large storms resulted in the failure of portions of the original restoration work. The project is in a rural area and the surrounding land use is
primarily agriculture and forest.
4b. Have Corps permits or DWR certifications been obtained for this project (including all prior phases) in the past? *
Yes No Unknown
4f. List the total estimated acreage of all existing wetlands on the property:
0
4g. List the total estimated linear feet of all existing streams on the property:
3800
4h. Explain the purpose of the proposed project: *
The project purpose is the restoration of approximately 300 linear feet of stream to repair bank failures from a previous stream restoration. The project will regrade and stabilize a failed
bank adjacent to a barn and further stabilize a stream ford to prevent future erosion.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: *
To accomplish the stream restoration project, grade control structures will be installed, eroding banks will be stabilized and stream channel dimensions will be adjusted to ensure long-term
stability. A typical pump -around system shall be used during all stream remediation efforts to ensure that all stream work can be conducted in dry conditions. Standard construction
equipment such as a tracked excavator, skid steer and dump truck will be used to complete this project. At the conclusion of the construction, all disturbed areas shall be planted and
seeded to ensure a healthy stand of vegetation.
5. Jurisdictional Determinations
5a. Have the wetlands or streams been delineated on the property or proposed impact areas?*
Yes No
Comments:
5b. If the Corps made a jurisdictional determination, what type of determination was made?*
Preliminary Approved Not Verified Unknown N/A
Corps AID Number:
5c. If 5a is yes, who delineated the jurisdictional areas?

6. Future Project Plans
6a. Is this a phased project?*
Yes No
Unknown
Are any other NWP(s), regional general permit(s), or individual permits(s) used, or intended to be used, to authorize any part of the proposed project or related activity?
