2.0 INTRODUCTION
This report details the abandonment of an unutilized groundwater monitoring well identified as EMW-
1 located on a vacant property with a parcel ID 20118102. The address for the property is 9801
John Price Road, Charlotte, NC 28273. The unutilized groundwater monitoring well was discovered
on the property that was previously identified in a Phase II ESA,dated September 6, 2019 by SITECH
Consulting, P.C.,as EMW-1. This report serves as the required certification for the well
abandonment. The property and monitoring well location is depicted on Attachment 1.
3.0 MONITORING WELL ABANDONMENT
The well abandonment was completed on October 7, 2022 by Innovative Environmental
Technologies(IET),a North Carolina Certified Well Contractor#3455-A. The well abandonment
record (form GW-30) provided by IET is in Attachment 2. The abandonment was completed under
the supervision of Jared Hamela, a North Carolina Licensed Geologist.
The abandonment included grouting the interior of the well casing pipe using a tremied neat cement
with a mixture ratio of 94 pounds of Portland cement to 7 gallons of water. The well casing pipe was
removed to below ground surface and the well cover and pad were removed. A photographic log of
the abandonment is provided as Attachment 3.
