Dear Mayor Langford:
This letter is in reference to your permit renewal request received June 7, 2022.
In a letter dated August 24, 2022, Leah Parente -
Discharge Branch asked you to provide additional information (copy of letter attached) no later than the
close of business on September 26, 2022. As of today, this information has not been received, therefore,
the Division of Water Resources must return your application as incomplete in accordance with 15A NCAC
02T .0107(e)(2).
Please be advised that operation of the subject facilities without a valid permit is a violation
of G.S. 143-215.1, and may subject you to appropriate enforcement actions pursuant to G.S. 143-
215.6A., 6B., and 6C. Civil penalties of up to $25,000 per day per violation may be assessed for failure
to secure a valid permit.
Therefore, it is imperative that the requested additional information and associated renewal
application be submitted within 30 days of receipt of this letter. Failure to provide the requested additional
information and the associated renewal application shall result in a Notice of Violation and may include a
civil penalty assessment.

Division of WaterResources’ Central and Regional staff has reviewedthe application package
received June 7, 2022. However, additional information is required before the review may be completed.
Please address theitems on the attached pages no later than the close of business on September 26, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding items
in SectionsA through E, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2).

A.Operator in Responsible Charge (ORC):
1. The Operator in Responsible Charge (ORC) listed for the Town of Seaboard WWTF facility in the
submitted application is Jeffrey Carl Long Sr. (SI/993135). However, Mr. Long has not been
designated as the ORC for this facility. The ORC designated for this facility is currently listed as
Rebecca L. Turner (SI/23933). Please confirm that Ms. Turner is no longer associated with your
facility. Additionally, a Water Pollution Control System Operator Designation Form (attached)
must be submitted designating Jeffrey Carl Long Sr. as the ORC for this facility. It is important to
note that the Non-Discharge Branch does not process this form. Please submit the form to the Water
Pollution Control System Operators Certification Commission (WPCSOCC) by mail, fax, or email

2. Johnny G. Young (SI/23129) is currently listed as the backup ORC for the Town of Seaboard
WWTF facility. Please confirm that Mr. Young is still associated with the facility in this role. If
Mr. Young is no longer associated with this facility, please designate a new backup ORC on the
Water Pollution Control System Operator Designation form described above.
B. Facility Modifications:
1. Chris Smith (NCDEQ Division of Water Resources, Raleigh Regional Office) spoke over the phone
with the Seaboard Town Clerk regarding the modifications to the Town of Seaboard WWTF facility
that were permitted on July 15, 2021. During that conversation, the Town Clerk stated that none of
the modifications listed in Permit WQ0003299 Version 4.1 have been completed; however, work
on these modifications is scheduled to begin in the next thirty (30) days. Please confirm that these
statements are accurate.
C. Monitoring Wells:
1. The application submitted for this facility indicated that monitoring wells MW-6, MW-7, MW-8,
MW-9, and MW-10 are inactive. Attachment C in Permit WQ0003299 Version 4.1 indicates that
these wells are active and require regular sampling throughout the permit cycle. Please provide
additional information regarding the status of these wells.
D. Open-Atmosphere Structures:
1. The application submitted for this facility indicated that the capacity of the storage lagoon is
4,425,000 gallons. Our records indicate the capacity of this lagoon is 7,425,000 gallons. Please
provide additional information regarding the volume of the storage lagoon including any
modifications made to the lagoon.

E.Permit Schedules:
1. Condition I.4 of permit WQ0003299 Version 4.1 states: “The Permittee shall investigate additional
areas for wastewater irrigation disposal in the event that improvements gained as a result of the
Infiltration and Inflow (I&I) study and improvements from grant funding fail to bring average daily
flows within compliance limits and meet irrigation limitations and report findings to the Raleigh
Regional Office. \[15A NCAC 02T .0108(b)(1)(B)\]”. Please provide additional information
regarding the status of the investigation into additional areas for wastewater irrigation disposal.
