Dear Ms. Euliss,
The North Carolina Department of Transportation is scheduled to replace Bridge #38 with a new bridge
at the same location. Because the project is in the Jordan Lake watershed, a buffer authorization is
required. I am requesting review through the Low 7mpact Bridge Process. I have included the
Attachment L with a project description, Attachment G and a set of the project plans. Please review this
project far authorization by your Division.
We plan to begin construction as soon as possible. If further information is required, please contact Jerry

Project Description Description — The project consists of replacing Bridge #38 over Beaver
Creek on NC 61 in Guilford County and improving roadway approaches. The existing 41' L X
23' W bridge is structurally deficient and will be replaced with a new 100'L X 39'W bridge at the
same location. An off-site detour will be utilized during construction. No wetlands were
identified within nor adjacent to the project area. Buffer impacts are necessary to construct the
bridge to NCDOT standards. Proposed buffer impacts include 4402 sf of buffer within Zone 1
and 2054 sf of buffer within Zone 2 to accommodate the wider span and the necessary fill
slopes. The project also proposes 61 linear feet of permanent stream impacts for the purpose of

bank stabilization (riprap along the edge and not in thalweg of stream) which will be restricted to
just the banks beneath the bridge. Ten feet of temporary stream impacts are also proposed for
access and tying in of proposed riprap. Stormwater runoff from the bridge will be captured with
inlets and conveyed to the northeast side of the road where it will discharge to a preformed
scour hole outside of buffer zone two. A stormwater management plan is also attached. A
review was conducted for the presence of Bald Eagie and smail whorled pogonia, the only
federally listed protected species for Guilford County. An assessment within the review area
showed no habitat and review of NCNHP mapping showed no known occurences of either
species.

Review of HPO quad maps, IIPO GIS information, historic designations roster, and indexes was
undertaken on March 5, 2013. Based on this review, there are no existing NR, SL, LD, DE, or SS
properties in tlie Area of Potential EfFects, which is SO'from the centedine each way and 300' from each
end of the bridge. There are no structures within the APE and no historic structures within the vicinity of
the brid�e. The structures surrounding the project area are modern houses under fifty years of age. There
are no National Register listed or eligible properties within the APE and no survey is required. In
addition, Bridge No. 38 is not National Register eligible based on the NCDOT Historic Bridge Inventory.

project. Digital copies ofHPO's maps (I�unesville Quad��wgle) �s well as theHPOVJEB GIS Service
(http://b snccler.govlhpoweb/) wers last reviewed on Fxiday, March S, 2013. Tke�e are no tmown historic
arclutecturai resources located within the pro,ject area that may have iutaet arcfiaeological deposits within
the footpruit of the proposed project. In addition, topographic maps, lustoric maps (NCMaps website),
USDA soil smvey anaps, and. aerial photograptis were uiilized and inspected to gauge enviroumental

perspective, the APE consists prunarily of Chewacla sandy Ioatn (Ch) Followed by Mec]denburg sandy
clay loam, 2-6% siopes, eroded (M(iE32). Tlius, much of Uie project area is commonly floodeci for hrief
periods based on the presence of poorly draiued soils whereas the remaiuder lias been affected by erosion.
Moreover, mast of [he preject area is ca#egorized as a Flood Hazazd Zone, based on an overlay oP
fToodplaiu mappii�g. In addition, the Office of State Archzeaology (OSA) has reviewed one (I) project
witl�in the aic;iziry of Bridge No. 38, for which the probabili#y of containing nuact �chaeological
