Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General —When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.

returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.

Sanitary Sewer Collection System Overview
Overview:
The Sanitary Sewer Collection System(CS) is comprised of 147 miles of gravity sanitary sewer line constructed
of various materials i.e. vitrified clay(VC), cast iron(CIP), ductile iron(DIP),polyvinyl chloride(PVC), and
standard diameter ration(SDR) [26 and 35] pipe.
The gravity system dates back to the early 1920s with VC pipe and brick manhole and throughout the years the
system components have transitioned to concrete precast manhole and SDR pipe.
The Sanitary Sewer Collection System(CS) is also comprised of 10.4 miles of forced sewer mains from the lift
stations.
Design:
The City's system design basics are as follows: 8"or greater main sewer lines, concrete manholes, service
laterals or 4"PVC or greater with a double sweep clean out at the right of way. The clean out caps are flush
with the surface with a metallic insert for locating.
Gravity: The majority of the system is gravity, and is mainly located within two river basins utilizing the low
points of each basin to provide sufficient drainage.
Lift Station:
The gravity system has four lift stations (LS)that pump into it; 601 Lift Station, Airport Lift Station, Laurel
Spring Lift Station and West Pine Street Lift Station.
Operation and Maintenance:
The Utilities Maintenance Division(UMD)maintains the collection system, i.e. installing new services,
repairing existing services, installing main lines and repairing existing main lines,jetting/cleaning main lines
and service lines, CCTV of all service lines and main lines,perform visual inspections of ROW,high priority
lines, general observation, and odor complaints.
The UMD is made up of 1 supervisor and 2 crews containing 1 crew leader, and 3 crew members. These crews
are also responsible for the water distribution system maintenance. The UMD has 2 crew trucks which are
equipped with various tools and supplies to respond to any issue with the CS. The crew leader and crew are on
call 24/7 to respond after hours to blockages, S SOs, or any other issues. The crews rotate weeks of being on
call. However, in the event of an SSO,the crew leader will contact the closet crew members for a quicker
response(see ERP).
The UMD also has a Jet Vac truck combo, Jet truck, a jet trailer, CCTV van along with 2 back hoes, 1 track
mini hoe, 2 dump trucks and other support vehicles, (4x4 pick-ups, 4x4 tractors, ATVs).
Supplies:
The UMD maintains a large amount of supplies for repair, new installations, emergency response, and clean up.
ORC:
The primary ORC is Ricky Dean,Utilities Maintenance Supervisor whom holds a Grade IV Collection System
Certification by the NC Water Pollution Control System Operators Certification Commission.
Rights of Ways (ROW):
The UMD budgets $30,000 annually to mow CS ROWs in the winter which is done by a contractor. The UMD
budgets $10,800 annually for spraying which is done by a contractor. The contractor that does the spraying,
sprays everything that was mowed with herbicide to deter brushy growth. The City tries to mow and treat 1/3 of
the system per year, with treatments warrantied for 3 years.
Root Control:
The City of Mount Airy hires a contractor to treat roughly 10,000 feet of sewer mains for root intrusion. This is
an annual cost of$17,500. 00 and holds a 3-year warranty as well.
Mapping:
The City of Mount Airy has an exclusive digital map of the collection system. The City also has various other
maps and construction drawings housed at the Public Works Department. All of the lines and supporting
infrastructure have been GPS located and mapped on the main City server.

The first two of these LSs are monitored with SCDA systems, which reports to the Wastewater Treatment
Facility. The UMD inspects the LS weekly and responds to any alarms or issues.
The second two have auto dialer SCDA that dial Police Station first, then WWTP.

The City UMD has designated all the HP lines and listed them by river basin. These are checked semi-annually

V1.2 Response Action Plan
SSO (SANITARY SEWER OVERFLOW) INCIDENT ACTION PLAN
1. The person receiving a report of an SSO (sanitary sewer overflow) shall obtain the flowing information
from the caller:
a. Phone number caller can be called back at if necessary.
b. SSO incident location.
c. Approximate time SSO has been occurring.
d. Approximate size of area affected if possible.
2. The person receiving a report of an SSO shall then notify dispatcher(person answering
phones at Public Works).
3. The dispatcher shall immediately page both U M (Utilities Maintenance) Crew Leaders and
the U M Supervisor and verify the information in#1 above has been obtained.
(See attached SSO Emergency Call Sheet.)
4. U M personnel shall call the dispatcher back to ensure they received the page and then begin
their response to assess and address the problem.
5. If UM personnel have not called the dispatcher back in 15 minutes the dispatcher shall
notify the following Engineering personnel in order as listed: Engineering Technicians,
City Engineer, and Public Works Director. The dispatcher shall also activate the U M
Group Page number notifying all U M personnel at once, while the Engineering personnel
is attempting contact individually and assessing the SSO.
(See attached SSO Emergency Call Sheet.)
6. The first person on scene at the SSO shall be IC (Incident Commander), the IC position
may be assumed by or reassigned by the supervisor arriving on scene. The IC will evaluate
the overall situation to determine if City crews can address problem or if a contractor needs
to be brought on scene for assistance.
7. If a contractor's assistance is required the IC shall first notify Public Works Director
or City Engineer in Public Works Directors absence before calling contractors in order as
listed on the attached SSO Emergency Call Sheet.
8. The IC or his designee shall complete Sewage Spill Checklist and comply with notifications
to NCDENR/NCDWQ or NC Emergency Management as indicated on the SSO
Emergency Call Sheet and Sewer Spills Notification sheet within 24 hours.
9. The CS—ORC (Collection System—Operator Responsible in Charge) shall visit and
