For each oudaA, provide an estimate of the area (include units) of imperious surfaces (including paved areas and builWng moos) drained to the outfall and an estimate of the total surface arae drained by the outlall. Outfall Area of Impervious Surface Total Area Dished Outfall Area of Impervious Surface Total Area Drained Number (provide units) (provide urns) Number (Pmvde units) (provide units) S14-001 0 acre 10 acres SW -002 1 acre 9 acres SW -003 0 acre 2 acres SW -008 0 acre S6 acres S. Provide a narrative description of significant materials that are currently or in the past three years have been treated, stored or disposed in a manner to allow exposure to storm water, method of treatment, storage, or disposal; past and present materials management practices employed to minimize contact by these materials with storm water runoff; materials loading and access areas, and the location, manner. and frequency in which pesticides, herbicides, soil conditioners, and fertilizers are applied. outfalls Sw-mol and SW -Doe would discharge storm water from abandoned ash ponds that have not been used for more than 35 years. These areae are now covered with vegetation. Previously, NCDHNR concurred that it would take greater than a 25 -year storm event to have storm water diacharge from the existing stand pipes in these inactive ash ponds. Outfall SW -002 discharges storm water from along the east side of a railroad spur that formerly received coal shipments to the plant. All coal has been removed from site. Two pipes direct flow from this leas than one acre area into a field. A collection point within the field discharges into Shaddox Creek. Approximately one acre of impervious surface, including a roadway and a gravelled employee parking lot, also drain to this irllection point. No herbicides nor fertilizers are applied to the field. Outfall SW -003 discharges storm water from along the south side of the entrance rued. This small, grass -lined area is not located near any industrial processes at the plant site nor is it located near any portion of the Bite where coal or other chemical are unloaded. No herbicides nor fertilizers are applied in this area. C. For each outfall. provide the location and a description of existing structural and nonstructural control measures to reduce pollutants In storm water runoff; and a description of the treatment the storm water receives, including the schedule and type of maintenance for control and treatment measures and the ultimata disposal of any solid or fluid wastes other than by discharge. I certify under penalty of law hat the outfall(s) covered by this application have been tested or evaluated for the presence of nonstonnwater discharges,7thatall nonstormwater discharged from these oultall(s) are identified in either an accompanying Form 2C or From 2E application for the outfall. Name and Official Tide (type or print) Signature Date Signed Henry Gales, DRC ' B. Provide a description of the method used, the date of any testing, and the onsite drainage points that were directly observed during a test. Previously, when the SW outfalls were permitted, the Cape Fear Plant ORC conducted an annual a visual inspection of all storm water outfalls during an extended period of dry weather. This visual inspection served to ensure that there are no nonatorm water discharges from the permitted storm water outfalls. Certification statements were signed, dated, and maintained onsite. I. Significant leaks or Spills Provide existing Information regarding the history of significant leaks or spills of toxic or hazardous pollutants at the facility in the last three years, including the approiamate date and location of the spill or leak, and the type and amount of material released_ here have been no leaks or spills of toxic or hazardous pollutants at the facilty Ln the last three years.
