ICA/PCN Comments:
A PCN is being issued due to sediment loss and turbidity in stream channel.Art King, DEO was notified and planned to visit the site
this afternoon. Please notify this office once the agencies have reviewed site and have made recommendations m restore site.
Remarks and Recommendations:
Talked to Tracy|ngo|d. P|while on the project and talked to John Olinger, DCE and Travis Morgan, RE in Division office.
Project is out of compliance duo to failure to take all reasonable measures to prevent off-site sodimontadon,and failure to fv||mw
approved plan.At time of review,the contractor was not utilizing proper BMP's in construction of stream restoration channel.
Contractor had removed impervious dikes upstream and down stream and pump around and was completing removal of signifioava
soil plug from a live stream channel.As indicated on last month's o.o.report and per permit conditions,"Channel/stream relocations
shall be constructed in a dry work area and stabi|izod,and approved by DVVR prior to diverting water into the new ohanno|" Please
note the following corrective actions:
1)Since majority of soil material has been removed from channel,need to stabilize remaining disturbed stream channel banks with
oo|rfibor mauing,asap,ensuring matting is instaUod,as per detail. Before pn400t completion,ensure live stakes are driven into stream
banks,as per detail.
2)Since constructed wetlands are disturbed and water has been turned into channel,recommend installing silt fence w/wattle outlets
along stream channel,asap. Need to rip wetlands,as per spec.and seed with native seed mix and mulch.
3)Need to repair washes on existing stream banks caused by recent rainfall and remove sodimont,as per DVVO recommendations.
Additional ooir fiber matting may bo needed adjacent to stream channel,depending on concentrated water flow from wetland tonew
channel.
4)Ensure silt fence is installed around headwall area of new culvert and sood/matdismrbod areas on slope.
AJso,need m address the following before project acceptance.

seed/mulch disturbed areas or temp.seed/mulch to meet NCGO1 seeding requirements.#~Gta 17+00 Lt.on-Y2'need tostabilize
headwall area at outlet end of pipe and repair seed/mat weakly vegetated slope.

NPDEG documentation was not reviewed.Continue to perform inspections on a weekly basis and after.5"rainfall event within 24
hrs.Also,ensure o.o.plans are updated and initialed on daily basis.
