I found the spread sheet Storm EZ and started completing. Please review tab 2. It says I have 9% impervious. I'm not sure I am completing the form correctly. However, when I had a chance to review the package, I realized that our staff had sent me another copy of the same package we received on August 8 t that I had reviewed. The plan deficiencies was the reason for the October 13th "Request for Additional Information." Did you want to talk tome about the project before you submitted the" additional information" that the Karen Higgins October 13th letter requested? If so, I did not understand that and I will be very glad to talk to you at any time. I understand that you are employed by DOT during the day so don't hesitate to call me on my cell when I don't answer my office phone. Nights or weekends will also be fine. I don't want the project to fall behind schedule because of any miscommunications. If you wanted to ask about using a Stormtech system, we certainly will consider those. However, we could not tell from the "Storm Water Plan" sheet that we received on August 8th if you plan to use them as a detention system or an infiltration system. We did not receive any stormwater calculations with the package you submitted in August. Did we miss that or will that be coming later? Site layout showing all built -upon areas, drainage features, stormwater collection systems (inlets, outlets, swales, pipe elevations, slopes and diameters), and SCMs at ultimate build -out. g. Existing contours, proposed contours, spot elevations, and finished floor elevations. h. Existing drainage (including off - site), drainage easements, pipe sizes, runoff calculations. i. Drainage area boundaries (included in the main set of plans, not as a separate document). j. Existing and proposed maintenance access routes, utility easements, drainage easements, public rights -of -way etc. k. A construction sequence that shows how the SCMs will be protected from sediment until the entire drainage area is stabilized. 6 Two sets of sealed, signed, & dated full -sized plan details of each SCM (or typical if appropriate) in both plan view and cross - section at a scale of 1" _ 30' or larger, including: a. Dimensions, side slopes, length to width ratios and elevations with a benchmark for clean -out if appropriate. b. All applicable conveyance devices, including: bypass structure, pretreatment area, flow distribution device, underdrains, outlet device, outlet dissipater and level spreader. c. A table of elevations, incremental volumes and accumulated volumes (if applicable). d. The locations of each species and a planting detail. b. A table summarizing the size, spacing and total quantity of each plant species that should be used. c. The source nursery for the plants. d. A one -time fertilizer specification to establish plants if necessary. 8 A soils report based on field evaluation indicating the SHWT elevations within the footprints of the proposed SCM(s). Provide a map of the boring locations with the existing elevations and boring logs. For infiltration SCMs, the report shall include the soil type, infiltration rate and method for determining the infiltration rate. Soils information shall be signed and sealed 9 A copy of the signed and notarized operation and maintenance (O & M) agreement. 10 A copy of the proposed deed restrictions protecting the SCMs and limiting the built -upon area so that it does not exceed the capacity of the SCM(s) or the high density threshold (as applicable).
