Inspection Summary:
Tricia Lowery of the NC Division of Water Resources, Winston-Salem Regional Office conducted a compliance evaluation
inspection of Salem Glenn collection system (WQCSD0194) on 10/4/2022. Sam Pegram (collection system manager) was
present for the inspection. This is a private community collection system. The collection system contains 6 pump stations,
with —4.5 miles of gravity sewer and —3.25 miles of force main. Results of the inspection are summarized in the following
paragraphs:
15A NCAC 02T .0403 Permitting By Regulation
(a)(1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface
waters and to prevent any contravention of groundwater standards or surface water standards.
Observations: Overall, Mr. Pegram operates and maintains the collection system well.
Compliant: Yes
(a)(2) A map of the collection system shall have been developed and shall be maintained.
Observations: Map of collection system is adequate. Pipe age and materials are not listed on map, however, Mr. Pegram
has documentation as a supplement to map with pipe age and material.
Complaint: Yes
(a)(3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule,
spare parts inventory, and overflow response shall have been developed and implemented.
Observations: Overall, all documentation pertaining to the maintenance and operations of collection system are in order and
satisfactory. The only issue found was in the spill response plan (overflow response). The spill response plan should
include a response time to events, a source of emergency funds, and plan for site sanitation with a list of clean up materials.
These are issues that can be easily remedied with slight revisions to spill response plan.
Compliant: Overall yes. Please revise spill response plan.
(a)(4) Pump stations that are not connected to telemetry system shall be inspected by the permittee or its representative
every day, 365 days per year, unless the permittee demonstrates that the daily inspections are not necessary because the
pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection
interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry
system shall be inspected once per week.
Observations: All pump stations are connected to telemetry, therefore are inspected once per week.
Compliant: Yes
Special note regarding pump station inspection:
Keswick pump station was inspected. The pump station did not have a pump contingency plan posted, nor was the audible
alarm at proper volume. Thick tall shrubbery encases the pump station; therefore, they must cut an opening to reach the
pump station, which might pose security issues at pump station. It is important to note, Mr. Pegram has locks on both the
well and control module, so pump station components are secured.
Additionally, the tall thick shrubbery obscures the pump station identifier sign. At the time of inspection, we had some
difficulty moving through a small opening in the shrubs to reach pump station. This raises concerns of emergency
equipment access to pump station. There are no permanent generators at pump station, consequently they must rely on
portable generators in case of power failure.
Logbook records amps but does not document the lubrication of pump nor inspecting and exercising of valves. It does
document testing of alarms and telemetry communication. Mr. Pegram test alarms by lifting float from pump station.
Inspector suggests raising the audible alarm and conduct landscaping as to not obscure identifier sign and make it possible
for emergency equipment to reach pump station in emergency situations. Inspector does acknowledge the residents may
be concerned over louder audible alarm and visibility of pump station and therefore be the reason these issues exist.
(a)(5) High -priority sewers shall be inspected by the permittee or its representative once every six months, and inspections
shall be documented.
Observations: Only one high -priority line in system. It run beneath the lake. Aqua NC contracts Subsurface to inspect,
flush and clean lines. Documentation of line cleaning is in order and no issues found during inspection.

(a)(6) A general observation by the permittee or its representative of the entire collection system shall be conducted once
per year.
Observations: Due to small and private nature of collection system, Mr. Pegram oversees entire collection system on a
regular basis and the documentation is adequate. This is satisfactory to meet annual inspection requirements

(a)(7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC
02B .0506(a), and public notice shall be provided as required by G.S. 143-215.1 C.
Observations: No SSOs reported over the past 3 years. The spill response plan does have appropriate phone numbers in
case an SSO should occur.

(a)(8)(B) Grease Control Program shall be in place for private owned collection systems that will include bi-annual
distribution of grease education materials to users of the collection system by the permittee or representative.
Observations: Grease control educational materials are distributed every 6 months to residents via utility bill.

(a)(9) Right-of-ways and easements shall be maintained in the full easement width for personnel and equipment
accessibility.
Observations: All Terrain Mowing is contracted to conduct maintenance of right of ways. Documentation is in order and no
issues found during inspection.

(a)(10) Documentation of compliance with Subparagraphs (a)(1) through (a)(9) of this Rule shall be maintained by the
collection system owner for three years with the exception of the map, which shall be maintained for the life of the system.

Comment: This is a private community collection system. There is a clubhouse in community, but Mr.
Pegram has not inspected it recently. FOG education is distributed every 6 months to

Comment: Operation and maintenance logs are satisfactory. No issues found. One HPL in system. Due
to small and private nature of collection system, Mr. Pegram oversees entire collection system
on a regular basis and the documentation is adequate. This is satisfactory to meet annual
inspection requirements. Good documentation of line cleaning and manhole inspections.
