I am reviewing the above referenced project, and in order to complete the review, some additional information is required:

1. This application appears to require an Individual Water Quality Certification under NWP 29. 401 applications require payment of an application fee, and a 30-day public notice. Sheri,
can you please send a fee request (minor) and send this project for public notice? Bryan, as the applicant, the fee request should come to your email address. [15A NCAC 02H .0502(a)(10)]
2. Please provide a copy of the Buffer Determination Letter. [15A NCAC 02H .0502(c)]
3. Please provide a site plan and impact map that includes wetlands, streams, and Zones 1 and 2 of the riparian buffer. The riparian buffer starts at the top of bank of the stream, so
the top of bank should be indicated on the maps. Please show the dimensions of impacts on these maps. The Division recommends hatching, shading, or coloring to help differentiate impacts.

7. Please submit updated Stream and Buffer Impact tables. You can use the tables in pages 10 & 12 of the attached form and return those two pages to me. Please list rip rap dissipators
