INSTRUCTIONS FOR FORM CSA 04-16 & SUPPORTING DOCUMENTATION
Documents shall be prepared in accordance with 15A NCAC 02T .0100, 15A NCAC 02T .0400, and all relevant
Division Policies. Failure to submit all required items will necessitate additional processing and review time.

General -. When submitting an application to the Municipal Permitting Unit, please use the following instructions as a
checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided
boxes will help produce a quicker review time and reduce the amount of requested additional information.
The Applicant shall submit one original and one copy of the application and supporting documentation.
The copy may be submitted in digital format.

16) application. Any unauthorized content changes to this form shall result in the application package being
returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long
as the attachments are numbered to correspond to the section and item to which they refer.

Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is
authorized to hold the utility franchise for the area to be served by the wastewater collection system, or

Capital Improvement Plan
FY 2018/2019- FY 2022/2023
PURPOSE AND DEFINITIONS:
The purpose of the capital improvement plan (CIP) is to forecast and match projected revenues and major
capital needs over a five-year period. Capital planning is an important management tool that strengthens the
linkages between community infrastructure needs and the financial capacity of the Town.
The Town defines CIP capital expenditures as any expenditure of major value that recurs irregularly, results
in the acquisition of a fixed asset, and has a useful life greater than one (I) year. As a rule of thumb, items
included in the CIP include those which:
• Involve acquisition, renovation, and/or construction of a single fixed asset greater than
$2,500.
• Involve any Iand purchases greater than $50,000 not associated with or included in another
CIP project.
CAPITAL IMPROVEMENT PLAN DEVELOPMENT:
The CIP is updated annually as part of the Town's regular budget process. Projects are reviewed and
evaluated based on Manager and Board of Commissioners' service desires, other Town infrastructure needs,
the financial capacity of the Town and the impact the projects will create on the Town's operating budget.
Once the projects are evaluated, a recommended CIP is developed that identifies the selection and timing
of capital projects into future fiscal years. First -year projects are incorporated into the Manager's
recommended annual operating budget. The Mayor and Board of Commissioners are also presented the
future, unappropriated planning years for their consideration, review and endorsement so staff can proceed
with planning and evaluation of potential capital projects.
CAPITAL IMPROVEMENT PLAN:
The capital improvement plan is simply that - a plan. As such, projects are subject to change based on
new or shifting service needs, special financing opportunities, emergency needs, or other directives or
priorities established by the Manager or Board of Commissioners. Because priorities can change, projects
included in outward planning years are not guaranteed for funding.
The Town of Farmville's CIP achieves five major objectives as a component of the Town's budget and
financial planning process:
I. Helps the Town rationally and intelligently plan for the repair, replacement, and acquisition of
capital items that are necessary in providing high quality services to the citizens of Farmville.
2. Assists in fiscal planning by forecasting capital demands together with future revenues and
expenditures (as part of the financial plan).
3. Insuring better coordination, evaluation, and planning of projects to serve the community
and its' needs.
4. The CIF, together with the Financial Plan, serves as a guide to decision -making for the Board,
Manager and employees.
5. The systematic and comprehensive analysis of capital needs increases the probability of
making rational (and correct) budgetary judgments since improvements are identified,
prioritized, and matched to the projected fiscal resources of the Town.
The Town of Farmville operates a wastewater collection system that serves approximately 5,000
town customers and the towns of Walstonburg and Fountain. The operation and maintenance of
the collection system is regulated by Permit No. WQCS00069 issued by the Department of
Environment and Natural Resources. The collection system is composed of approximately 39
miles of various sizes of gravity sewer lines and nine miles of force mains. There are 13 pump
stations in operation.
CIP Projects - Summary
Service Truck - The Utility Department has a staff of six and five service trucks. As the trunks
reaches 100,000 miles they are scheduled for replacement. The Utility Department is responsible
for checking the collection system on a daily basis, responding to emergencies and performing all
maintenance and repairs. Reliable transportation is essential.
Sewer Combination Truck - Debris, such as grease, solids and nonbiodegradables, accumulate
in the pump stations and sewer lines. If not removed, these solids can interfere with the proper
operation of the collection system leading to blockages and sewer spills. The Vacuum Truck will
be used to remove these accumulated solids from the system.
PS Bypass Pumps - Pump stations in the collection system are subject to failure due to
mechanical or electrical problems. During a failure, a bypass pump allows the station to continue
in operation, without a sewer spill, while repairs can be made.
PS Motor Repair The dry well Johnsonville pump station flooded which submerged the pump
motors. The motors were send to motor repair shop.
Bioaugurnentation Treatment - The sewer collection system is subject to the accumulation of
large amounts of grease for both commercial and residential users. Grease is the number cause of
dry weather sewer spills. Bioaugmentation retards the accumulation of grease buildup in both the
sewer lines and pump station wetwells.
Predictive/Preventative Pump Evaluation - The sewer collection system has 12 pump stations
with two pumps in each pump station. The continuous operation of the pumps are essential to
prevent sewer spills and sewer backups. Proper maintenance of the pumps will also reduce
emergency response calls. A predictive/preventative evaluation program will reduce the number
of pump failures, extent the useful life of the motors and help prevent sewer spills.
Gas Monitor - The sewer collection system numerous confined spaces. These confined spaces
are considered to have the potential for hazardous atmospheres. OSHA requires that these areas be
tested used a gas monitor prior to and during entry by staff.
Chemical Root Control - The sewer collection system has 39 miles of pipe. Pipe joints or breaks
in the pipe can allow for roots to grow in the pipes, These mats of roots cause sewer blockages by
trapping solids or just by their growth. Cutting the roots outs provide a temporary solution, but
regrow more vigorously after cutting, requiring staff to make repeated service calls to remove the
roots. Chemical root control provides a more long term solution for root removal.
The Town of Farmville recognizes that we have deficiencies on the 24" outfall line located at
Middle Swamp. We are in the engineering process of replacing this line.

All Town Employees
Sanitary Sewer Overflow (SSO) Response Procedures
1. Sanitary Sewer Overflows endanger our environment and potentially the health of
any citizens, or employees that come into contact with an SSO. Unchecked SSO's
can result in significant penalties from State and Federal environmental regulatory
agencies as well. The procedures below were designed to protect our
environment, citizens, and employees from an SSO event. These procedures apply
to all Town personnel.
2. Upon discovery or report of an SSO notify the Town Hall immediately. If it is
after regular work hours, notify the public works employee on call, Public Works
Director, and the Town Manager immediately. If for some reason the public
works employee on call can not be reached, notify the Public Works Director
immediately. The Town Employees required to respond to the Overflow shall
report to the scene within 60 minutes.
3. Upon arrival, the public works employee will make an immediate assessment to
determine if a SSO event has taken place. If an event has taken place, or is in
progress, the employee will attempt to eliminate the source of the SSO and contain
any spillage that has taken place. The employee will also ensure that the Public
Works Director, Town Manager, and any other resources or manpower as needed
to eliminate the blockage and contain the SSO have been contacted.
4. In the event that the Public Works Department needs assistance from other Towns the
Town Manager is to be contacted immediately and informed of the situation and the
type of assistance needed for approval.
5. Please review and familiarize yourself with the attached Sanitary Sewer Overflow
(SSO) Procedures Manual and contact list. In the event we have a large SSO we will
need assistance from other departments in Town.
PURPOSE:
The purpose of this document is to protect the citizens of Farmville, North Carolina
and the environment by establishment by establishing written procedures for
responding to Sanitary Sewer Overflows (SSO). This procedure applies to all Public
Works employees that in the course of their work may deal with SSO's. These
procedures will be reviewed at least semi-annually, or whenever revised guidance
from county, state, or federal authorities dictate a revision is necessary.
SSO REPORTING PROCEEDURES
Upon discovery or report of a possible SSO site during normal duty hours,
notify the Public Works Director by telephone, pager, or radio. Then notify the
Town Hall.
2. After normal duty hours, notify the public works employee on call
immediately. If for some reason the public works employee on call can not be
reached, notify the Public Works Director immediately.
3. In the event the employee on site requires assistance from other departments
within the town or other towns, the Town Manager is to be contacted and
informed of the situation and the type of assistance needed.
4. The ORC or the Town Manager will be the primary interface between the Town
of Farmville and the State of North Carolina Division of Water Quality and the
Pitt County Health Department on all SSO matters.
5. The ORC will report overflows from any sewer line or pump station, or the
bypass of any wastewater treatment system or any component thereof to the
North Carolina Division of Water Quality. This requirement applies in the
following cases:
a. Any spill of 1,000 gallons of more; and
b. Any spill, regardless of the volume, if any waste reaches the surface
waters of the State. Please note that conveyances such as drainage
ditches and storm sewers are considered waters of the State.
c. The ORC is required to provide an oral report to Regional Office as
soon as possible, but in no case later than 24 hours after the spill.
d. If the spill occurs after normal business hours, the event must be
reported the next working day. Should a spill occur after hours which
reaches the surface waters and for it may be considered as either an
environmental or health hazard, the report must be made immediately to
the Division of Emergency Management at 1-800-858-0368. Their
office will contact the appropriate Division of Water Quality Staff.
e. A written report (see attached form) must be submitted to the Regional
Office within five (5) business days of the time that the Town of
Farmville becomes aware of the spill occurrence. All parts of the form
must be completed in detail and submitted either at the time of the oral
report or within the required five (5) days. It is highly recommended that
the form be faxed to the regional office, and then followed up by a
telephone call to confirm that the fax has been received and to provide
any additional information that may be needed.
If a discharge/spill of 1,000 gallons or more reaches surface waters, a
press release must be completed within 24 hours of the spill/discharge.
This press release must be to all newspapers to provide general coverage
within the county.
g. If a discharge/spill of greater than 15,000 gallons occurs, a public notice
is required along with a press release. Public notice must be given to
persons in Pitt County and the county downstream from the
discharge/spill.

CONTAINMENT PROCEDURES (All SSO's)
1 As soon as the employee arrives on the site of the SSO their first priority will be
to contain the spill as to not allow it to reach surface waters. If there is no
danger of the SSO reaching surface waters, contain the SSO to as small an area
as possible.
2. The second priority shall be to tape off the SSO area to prevent any unauthorized
person from entering the contaminated area. This includes the banks of surface
waters that the SSO has reached.
3. Containment shall be accomplished in any number of ways including but not
limited to the construction of; ditches, ponds, or dams. The sewage contained in the
containment areas shall be pumped back into the sewer system or transported to the
WWTP by pumper trucks or vacuum trailers.
4. Trash pumps capable of handling the flow of the sewer line are to be placed at the
overflowing manhole and the sewage pumped into the next downstream manhole
until the blockage is corrected. One employee shall remain onsite until the pumps are
no longer needed.
BLOCKAGE CORRECTIVE PROCEEDURES
1 As soon as the SSO has been contained and a bypass pumping system has been set
up, attempt to eliminate the blockage by first checking for problems in the invert
of the overflowing manhole.
2. Utilizing the Town's equipment and outside contractors (if needed and approved by the
Town Manager) eliminate the blockage (see attached list of outside contractors).
Containment and bypass pumping are to remain operational until the blockage is
corrected.
CLEAN UP PROCEDURES
ALL SSO's
1. Remove all paper, plastic or any other non -decomposing material from the site.
Material should be bagged in a container clearly marked biohazard and transported
to the Wastewater Treatment Plant (WWTP).
ON ASPHALT OR CONCRETE
1 Set up containment as far away from the storm drainage system as possible to
prevent the spread of the spill taking extra care not to allow the spill enter the
storm drainage system.
2. Put up the warning tape around the SSO area.
3. Spread lime on spillage and sand (or equivalent material) to a depth of 1 inch. Let this
stand for one (1) hour and sweep up. If moisture or grease is still present, repeat the
procedure.
4. After removing the contaminated material, wash down the area with water and
Clorox. Pump the water and Clorox back into the sewer system. Transport the wet
sand to drying beds at the waste water treatment plant.
5. If the spill reaches the storm drainage system, refer to and follow the creek or
stream procedure.
CREEK OR STREAM
1. If possible a dam needs to be constructed near a manhole or at a location which
is accessible to a jet/vacuum truck. Build the dam using dirt that is capable of
handling creek or hydrant water without dam failure. Station two pumps at the
dam that is capable of handling the creek flow. Use one pump for backup. Have
a backhoe on site with extra material at all times so the dam can be
rebuilt/repaired should a breach occur. There will be two workers at the pump
site at all times during the pumping operations. Pump effluent from the dam
into the manhole.
2. Put up warning tape from the SSO site to the pump site on both sides of the creek or
stream in populated areas. In unpopulated areas restrict access where there is any
chance of humans coming in contact with the SSO.
3. Flush the stream with water from a sufficient number of hydrants to remove the
waste product from the creek. Have the Wastewater treatment plant operator pull
samples. Continue to pump and flush the stream until all fecal coliform tests come
back 500 colonies/ml or less. Pull samples every 24 hours until this reading is
obtained. Pull samples 100 feet above the SSO site, at the site, and at 400 feet
intervals until reaching the pump site.
4. If rip rap is in the creek, broom creek bottom and sides until there is no waste
residue remaining on, or within the rip rap structure.
5. If sludge is in the creek or stream, broom creek bottom and sides until there is no
waste residue remaining.
6. If the creek or stream flows into a body of water used for recreation, and or
drinking; notify the affected county and local officials as soon as the problem is
discovered. If this happens, refer to the pond and/or lake procedures.
POND OR LAKE
1. Contain the SSO area with a dam if possible.
2. Put up warning tape, black on red on the shoreline at the contaminated area.
3. In a pond, have the WWTP operator collect fecal coliform samples and
dissolved oxygen readings around the entire circumference. Set up pumping
operations below the dam. Fecal coliform samples should be taken two (2) feet
off of the bottom of the pond. Dissolved Oxygen readings are to be taken one (1)
foot under the surface of the water. Take equal sampling amounts around the
circumference of the structure.
4. When the first sampling results come in, contact the North Carolina Division of
Water Quality to determine the appropriate action to follow if the fecal coliform
amounts are greater than 8,000 colonies/ml.
5. Continue pumping until 500 colonies/ml or less are obtained in the structures.
6. Keep monitoring fecal coliform counts until the North Carolina Division of Water
Quality or the Pitt County Health Department say that monitoring is no longer
required.
OUTFALL LINE
1. Put up warning tape, "DANGER DO NOT ENTER," around the SSO site.
2. In open area, lime (powder) heavily and till. Repeat procedure. Seed and then put
down straw.
3. In an area where there is no undergrowth or small trees, wash area with large
amounts of water. Push towards the containment area and pump site. Do this until
no waste remains. Wait 12-24 hours and apply a heavy amount of lime to the
area. Do not lime when the weather forecast is predicting rainfall.
IN CITIZENS YARD
1. Till the area whenever possible. Apply lime, seed, and straw.
2. If you are not able to till, apply lime and two inches of topsoil and mix
thoroughly. Lime again lightly, then seed, and cover with straw.

The above listed lift stations have audible alarms and flashing lights to inform the town of any problems that may
occur at these stations. We also have signs posted at all lift station gates requesting that any citizen that may hear
or see the alarm, to call the Police Department at 252-753-4111. The Police Department will then page the
water/sewer person on call.
The lift stations and wet wells are checked on a daily basis as well as cleaning the grates. Daily records are
maintained on pump run times. These records include checking the audible alarm and light on a monthly
basis. The process requires that the pump be shut off and the wet well to fill up enough to set off the high
water alarm and lights. If a lift station alarm and lights fail to work properly, we can repair with parts on
hand or we order the needed parts by next day air. Purging the air lines has also been added to our monthly
records. The valves are exercised monthly and pumps are greased. The pump stations that have generators
permanently mounted are checked and ran weekly. (Johnson Field)
Pump Failure
In the event of a pump failure, the first this to do is to discover the problem. Depending on the problem, the
electricity should be off and lock out tag outs applied at electric box. The pump then can be torn down for
inspection.
We have a few parts for T3 and T4 pumps. A parts list will be provided and parts can be ordered and
received the next day. If needed, back up pumps at all lift stations are provided in a pump is down. A pump
that is down will be repaired and back in service as soon as possible. Records are maintained and lift station
incident reports are filed on pumps that have any kind of problem. All pumps are on alternate positions at all
times unless a pump has to be down.
Cleaning of Wet Wells
The Town of Farmville has started a cleaning practice of wet wells to be performed monthly. Wet well will be
flooded in order to remove grease and solids. The grease and solids are then disposed of properly. This practice
will be included in our records.
Generators
In case of a power outage, the Town of Farmville has two single phase, portable generators that will provide
power to Carolina Medical Products and Eaglebrook lift stations. Johnson Field ,Highland Mobile Home
Park ,and Watkins Mobile Home Park lift stations have a permanent mounted generator that comes on
automatically when there is a power outage. It is checked under load on a weekly basis. The Town owns one
large, portable generator that can alternate between the other four lift stations. The Housing Authority lift
station has a force main close enough that we can set up a sump pump and pump the wet well into the force
main. The Town also owns a vacuum truck that can access any Lift station in the town and pump out the wet
well, and then it can be dumped into a force main.
The Town of FannvilIe can handle a situation that calls for multiple simultaneous pumps to run during a
power outage. All portable generators and auxiliary equipment are ran and checked on a monthly basis.
