Dear Mr. Baker:
The Division of Water Quality (the Division) hereby issues the attached NPDES permit for the
subject facility. We issue this permit pursuant to the requirements of North Carolina General Statute
143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated October 15, 2007, or as subsequently amended.
Parameters of Concern — Changes to Your Permit. The Division acknowledges your comments
of May 9, in which you questioned the need for DWQ's permit Parameters of Concern and your
request to remove parameters from the permit. We offer the following summary explanation and
concessions:
• The parameters Total Residual Chlorine (TRC), Ammonia Nitrogen, and Total Fluoride
remain in the permit, but conditionally - monitoring required only if certain site conditions
exist [see permit Part I, Section A. (1.) and related footnotes].
• The parameters Total Calcium and Total Magnesium have been removed from this permit.
• The Division has relaxed the monitoring frequency for the remaining metals and toxicants in
your permit from 2/Monthly to Quarterly. However, continued monitoring of these
parameters is required by yours and all WTP permits statewide (see WTP Policy Sep2009
for facilities applying Conventional Technology).
• Please note that all samples are to be collected at your final outfall to the environment.
Future Reporting of Flow. Waynesville WTP is required under this permit to monitor flow, i.e.,
final wastewater discharge to Waters of the State consistent with NPDES permits statewide.
Moreover, the Division requires Continuous, Recording of flow for all WTP discharging greater than
0.0500 MGD. We understand from your application that your WTP processes generate intermittent

waste flow to 0.500 MGD, but that you currefitly do not have sufficient floWr -onitoring facilities to
accurately record your settling -pond effluent (final discharge to the environment). Understandably
therefore, we find no record over the past 5-year permit cycle of fmal-flow data, despite your
previous permit's requirement to report Instantaneous flow Weekly on your monthly Discharge
Monitoring Report (DMR) - a permit violation.
Monitoring and Sampling. As discussed above, this permit requires Continuous, Recording of
flow at the final outfall. However, for your convenience your compliance to this requirement will
not begin until approximately 18 months from the permit effective date, on February 1, 2013, [see
footnote 1, Section A. (1.)]. Until new facilities are installed, please monitor Instantaneous flow
rates Weekly and report these on your DMRs, and you are required to report the duration of each
discharge event so that the total volume may be calculated. Please be reminded that you must
sample effluent as it enters the environment, downstream of all treatment processes. If you have
questions or need assistance, please contact the Division's Asheville Regional Office (ARO) at 828-
299-7043.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable, you have the right to an adjudicatory hearing, upon written request submitted within
thirty (30) days after receiving this letter. Your request must take the form of a written petition
conforming to Chapter 150B of the North Carolina General Statutes, and you must file it with the
office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
Unless such a demand is made, this permit shall be final and binding.
This permit is not transferable except after notifying the Division of Water Quality. The Division
may require permit modification, or revocation and re -issuance. Please note that this permit does
not affect your legal obligation to obtain other permits required by the Division of Water Quality,
the Division of Land Resources, the Coastal Area Management Act, or other federal or local
governments.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended, the
Town of Waynesville
is hereby authorized to discharge wastewater from a facility located at the
Waynesville Water Treatment Plant (WTP)
341 Rocky Branch Road
Haywood County
to receiving waters designated as Allen Creek in the French Broad River Basin in accordance with
effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III and IV
hereof.
This permit shall become effective August 1, 2011.
This permit and authorization to discharge shall expire at midnight on January 31, 2016.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer effective.
Therefore, the exclusive authority to operate and discharge from this facility arises under the permit
conditions, requirements, terms, and provisions described herein.
Town of Waynesville
is hereby authorized to:
1. continue to operate an existing potable -water treatment plant (WTP) applying conventional
technology [including chlorine, aluminum sulfate, sodium hydroxide, hydrofuorosilicic acid,
orthophosphate (non -zinc), and calcium phosphate] utilizing a single settling basin
located at the Waynesville WTP, 341 Rocky Branch Road, south of Waynesville, Haywood County,
and
2. discharge filter -backwash wastewater from said treatment works via Outfall 001, a location specified
on the attached map, into Allen Creek [stream segment 5-16-7-(8.5)] a waterbody currently classified
C-Trout within subbasin 04-03-05 of the French Broad River basin.

1. Flow — the Permittee shall comply with Continuous, Recording by monitoring the final outfall to the environment
beginning on or before February 1, 2013. In the interim the Permittee may record Instantaneous flow Weekly.
2. Total Residual Chlorine (TRC) — Monitor only if the facility adds chlorine or chlorine derivatives to water that is
eventually discharged. The Division shall consider all effluent TRC values reported below 50µg/L to be compliant with
this permit. However, the Permittee shall continue to record and submit all values reported by a North Carolina -certified
laboratory (including field certified), even if these values fall below 50µg/L.
3. Total Fluoride - Monitor only if the facility back -washes with fluoridated fmish water.
4. Ammonia Nitrogen - Monitor only if facility uses chloramines [if NH3 is added to chlorinated water] prior to filter
back.
5. The Permittee shall monitor this parameter in conjunction with Whole Effluent Toxicity (WET) testing.
6. Chronic Toxicity WET Testing Quarterly P/F (Ceriodaphnia @ 90 %); see section A. (2.).
Condition: The Permittee shall discharge no floating solids or foam.

A. (2.) CHROi' C-TOXICITY MONITORING-- (QUARTERLY)
The Permittee shall conduct quarterly chronic toxicity tests using test procedures outlined in the "North Carolina
Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions. The
effluent concentration defined as treatment two in the procedure document is 90 %. The permit holder shall
perform quarterly monitoring using this procedure to establish compliance with the permit condition. The tests
will be performed during the months of March, June, September and December. Effluent sampling for this
testing shall be performed at the NPDES permitted fmal effluent discharge below all treatment processes.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the month in which it was performed, using the parameter code TGP3B.

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Section no later than 30
days after the end of the reporting period for which the report is made.
Test data shall be complete and accurate and include all supporting chemical/physical measurements performed
in association with the toxicity tests, as well as all dose/response data. Total residual chlorine of the effluent
toxicity sample must be measured and reported if chlorine is employed for disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required,
the Permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the
facility name, permit number, pipe number, county, 'and the month/year of the report with the notation of "No
Flow" in the comment area of the form. The report shall be submitted to the Environmental Sciences Section at
the address cited above.
Should the Pennittee fail to monitor during a month in which toxicity monitoring is required, then monthly
monitoring will begin immediately. Upon submission of a valid test, this monthly test requirement will revert to
quarterly in the months specified above.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of '
Water Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to
include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism
survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an
invalid test and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

Hi Cindy,
Joe Corporon is the permit writer for the Waynesville WTP. It does appear that their discharge is intermittent
(seasonal where in the summer they discharge once every 72-hours and winter once every 48-hours). Typical
discharge is 107000 gallons and about once a month 250000 gallons. The contact at the facility is Kyle Cook
(828) 456-8497. Kyle, please correct me if the above information is incorrect.
As we discussed, please let me know if an acute limit would be more applicable for this WTP.

Narrative Description of Sludge Management Plan
Filter backwash and sedimentation basin sludges are discharged to a lagoon for settling. For several
days monthly during warm weather a dredge pumps the consolidated material from the lagoon
bottom to tanker trucks which transport the sludge to the municipal sewer system. The sludge is
ultimately processed at the Town of Waynesville's Wastewater Treatment Plant.
