Ebron PO Box 368 ..Havelock, NC 28532 Subject: 401 Water Quality Certification Inspection City of Havelock - Culvert Replacement -Joe's Branch DWR Project # 2009 -0025 Dear Mr. Ebron: On November 6; 2014, Anthony Scarbraugh of the Division of Water Resources (DWR) conducted a follow up inspection for proposed wetland and stream impacts associated with replacement of 7 culverts at the subject property in Havelock, Craven County. Please note, a previous inspection conducted by DWR staff on September 2, 2014 revealed minor erosion along the culvert headwall along the stream crossings at Speight Avenue and Nunn Street. DWR staff contacted the City of Havelock and the required restoration was completed. This project was inspected for compliance with the General Water Quality Certifications (GCs) #3687 and 3688 with Additional Conditions issued on February 26, 2009 for the proposed project. The GCs approved impacts of 0.07 acres of wetlands and 308.9 linear feet of stream to complete your project as described in your application received by DWR on January 16, 2009. The inspection revealed that the project has been completed. Impacts did not exceed beyond the footprint of those approved. The inspection revealed that the inspected portion of this project is currently compliant with the GCs #3687 and 3688. As.a reminder, if you change your project you must notify us and you may be required to send us a new application. if the property is sold, the new owner must be given a copy of the Certification and approval letter and is thereby responsible for complying with all conditions. Upon completion of all work approved within the 401 WQC, and any subsequent modifications, the applicant is required to return the enclosed certificate of completion to the 401 & Buffer Permitting Unit, North Carolina Division of Water Resources, 1617 Mail Service Center, Raleigh, NC 27699 -1617. CONCURRENCE NEEDED Dear Mr. Quill: You have our approval, in accordance with the attached conditions, to temporarily impact 0.07 acres of wetlands and 308.9 linear feet of stream for the purpose of replacing 7 culverts as you described in your application received in full by the DWQ on January 16, 2009. After reviewing your application, we. have decided this fill is below the threshold for written authorization as long as all conditions of General Water Quality Certification Numbers 3687 and 3688 are met, which may be viewed on our website at http: //h2o.enr.state.nc.us /ncwe lands. This approval is only valid for the purpose and design that you described in your application except, as modified below. If you change your project, you must notify us and you may be required to send us a new application. If the property is sold, the new owner must be given a copy of this Certification and approval letter and is thereby responsible for complying with all conditions. If total wetland fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required as described in 15A NCAC 2H .0506 (h) (6) and (7). This approval shall expire when the corresponding Nationwide Permit expires or as otherwise provided in the General Certification.
