Hello Ms. Johnson,
The address on file with the Secretary for State is correct. The most recent permit for this project back in 2015 had the
address as PO Box 10801, Raleigh, NC 27605 (see attached permit for reference). The update form is so that we can
change the address on our end and get it to match what is on the Secretary of State; business updates on the SOS don't
get sent to our department.

I am not sure what you mean by the address on file with Secretary of State being different? Our business office is
correct on file with SOS. The project address is different, what address should we use on the application?

Hello Ms. Johnson,
The two forms you need to submit for this request are the permit renewal application form and the permit update
form, both of which have been attached to this email. The renewal application is the main form that needs to be filled
out; it has a $505 fee and needs to be notarized. The permit update form is a free form (still needs to be notarized) that
allows you to update items in the permit such addresses and points of contact. Given that the address on the NC
Secretary of State doesn't match what is currently on file, you would need to submit one as well.

2021. This is a reminder that permit renewal applications are due 180 days prior to their expiration per 15A
NCAC 2H.1045(3). North Carolina General Statutes and the Coastal Stormwater rules require that this property
be covered under a stormwater permit. Failure to maintain a permit subjects the owner to assessment of civil
penalties. More information about the Post -Construction State Stormwater Program can be found on the

Please submit a complete permit renewal form along with a $505.00 fee and other submittal requirements within
