5. Directions to Site: From Lenoir, take Highway 90 west to Collettsville.
Collettsville School is 500 feet NE of the intersection of Highway 90 and SR
1337 (Adako Rd). The outfall is on the south side of Highway 90 opposite the
school.
6. Discharge Point(s), List for all discharge points:

for any foreseeable needed expansion or upgrade.
8. Topography (relationship to flood plain included): The topography of the
area immediately surrounding the treatment plant is flat, low lying area
adjacent to the flood plain. The treatment plant does not appear to be
within flood plain.

Immediately below Collettsville School's discharge, current uses

basin, secondary clarifier, tablet chlorinator and chlorine contact tank.
An effluent lift station pumps treated wastewater to the Johns River.

hauled by Mull's Septic Tank and Concrete Company to Mull's septacre
disposal site (according to information provided by Mull's during a telephone
conversation on November 2, 1994).

PART IV - EVALUATION AND RECOMMENDATIONS
Upon verification that sludge is disposed of in accordance with NCGS 143-
215.1 and NCAC 2H.0200, reissue the permit.
In the applicant's cover letter for the permit application, he states, "If
sludge is removed, Mull's Septic Tank and Concrete Company of 2416 Mount Home
Church Road, Morganton, N. C. 28655 is contracted to do so. Mull's has a
permit with the state for disposal of sludge." During a telephone call to
Mull's on (November 2, 1994), this writer was told that all materials removed
from treatment plants serving the Caldwell County Schools (including
Collettsville) were transported to a farm and disposed of on Mull's permitted
site. Since Mull's does not hold a Land Application Permit from the EMC, the
site in question is assumed to be Mull's permitted septacte site, not a site
permitted for land application of wastewater sludges.
