Dear Mr. Bassinger:
Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act, or any other
Federal or Local governmental permits which may be required.

NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards
and regulations promulgated and adopted by the North Carolina Environmental Management
Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge wastewater from a facility located at the
Collettsville Elementary School WWTP
NC Highway 90
Collettsville
Caldwell County
to receiving waters designated as the Johns River in the Catawba River Basin in accordance with
effluent limitations, monitoring requirements, and other conditions set forth in Parts I, 11, III and IV
hereof.
This permit shall become effective March 1, 2005.
This permit and authorization to discharge shall expire at midnight on February 28, 2010.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Caldwell County Schools is hereby authorized to:
1. Continue to operate an existing 0.010 MGD wastewater treatment facility with the following
components:
• Influent bar screen
• Aeration basin
• Secondary clarifier
This facility is located at the Collettsville Elementary School WWTP in Collettsville on NC
Highway 90 in Caldwell County.
2. Discharge from said treatment works at the location specified on the attached map into the Johns
River, classified C waters in the Catawba River Basin.

Ms. Crawford,
We received a letter dated November 24, 2004 regarding the permit application submitted for
Collettsville Elementary School. You noted that a Total Residual Chlorine limit had been added to
the permit. Please see the information below that was received from our contractor, Mr. Ron
McClure, who oversees the plant at Collettsville Elementary School.
Per Ron McClure:
On the application for permit renewal, there is a part of the plant that used to be an old
chlorine contact chamber. This chamber is now used as an effluent distribution chamber.
We have not fed chlorine into this chamber for about 10 years. The plant now feeds
directly into Johns River rather than a small tributary of this river.
If this information is not sufficient, please let me know what else you need. This change will be
made on any future permit applications for this school.

I recently received a letter requesting one additional item that was needed in order to process our
permit renewal. The item is listed below:
• Solids handling and disposal. How often are solids (sludge) removed from your
facility? How are solids disposed after removal?
The system at Collettsville Elementary School is pumped out 2 times a year, removing
approximately 6000 gallons each time. These solids are removed to the City of Lenoir utility
department. All other septic tanks are pumped every 3 to 5 years or as needed.
