Dear Mr. Padgett
In accordance with your permit minor modification request received August 17, 2022, we are
forwarding herewith a modified Attachment A certified October 14, 2022, to the Town of Valdese for the
subject residuals management program.
Modifications to the subject Attachment A are as follows:
The hazardous determination monitoring requirement has been removed for the Lake Rhodhiss
WWTP as long as it remains the only residual source for this permit.
The modified Attachment A shall be effective from the date of certification until June 30, 2024
(i.e., expiration date of the current permit), shall replace Attachment A of Permit No. WQ0001990 issued
March 20, 2019, and shall be subject to the conditions and limitations as specified therein, as well as those
specified in the above-referenced permit. Please replace Attachment A of Permit No. WQ0001990 that
was issued March 20, 2019, with the modified Attachment A. Please note that the Permittee shall submit
a renewal application no later than January 2, 2024.
If any parts, requirements, or limitations contained in this certification are unacceptable, the
Permittee has the right to request an adjudicatory hearing upon written request within 30 days following
receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B
of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
