Yes X No Source of the Upset/Spill/Bypass (Location or Treatment Unit): 10" waste activated sludge cast iron pipe failure. Cause or Reason for the Upset/Spill/Bypass: Aeration basin 6 excavation for plant expansion was in close proximity to the waste activated sludge line that failed. After the heavy rains from tropical storm Ian, the cast iron pipe and the sloped wall for the excavation failed (unsure if one failure caused the other). Both failures led to waste activated sludge leaking into the excavation area. Spill/Bypass Reporting Form (August 2014) WWTP Upset , Spill, or Bypass 5-Day Reporting Form Page 2 Describe the Repairs Made or Actions Taken: All waste activated flow was cut off. Cleaning and camera work identified that the leak was due to failure of the step-1 WAS line. The step-1 WAS line was then isolated and all WAS flow from step-1 and step-2 was directed down the step-2 WAS line. Action Taken to Contain Spill, Clean Up and Remediate the Site (if applicable): All bypass flow was isolated to the excavation. Sump pumps were then used to pump contents of the excavation to the plant drain. Action Taken or Proposed to be Taken to Prevent Occurrences: The waste activated line that failed will be plugged and abandoned.
